Direct Support Professional – Pool Team Member

A Delta Community Support’s DSP Pool Team Member is a full-time DSP assigned to a specific County. This team member is paid for a minimum of 40 hours REGARDLESS of the actual hour worked. Additionally, the Team Member is eligible for all of the terrific Delta Community Supports benefits and perks.

· May work in the same Group Home all week.

· May work in a different Group Home each day of the week within the County.

· MUST BE AVAILABLE WHEN CALLED/SCHEDULED to work the Shift Hours the Team Member was hired to work!

Shifts Available: 7:00AM to 3:00PM; or 3:00PM to 11:00PM; or 11:00PM to 7:00AM

Delta Community Supports is seeking insightful and team-oriented employees to serve as a Direct Support Professionals reporting to the Residential Coordinator of Community Living. The identified candidate will support the individuals that we serve on a daily basis. This team member will join our organization at a critical time and assist with providing first-class service to our support recipients and other key stakeholders.

Position Summary

The Direct Support Professional provides direct care of older adults in a program setting which supports the interests and needs of individuals, often with disabilities, including intellectual disabilities.

Duties and Responsibilities

· Engage individuals in meaningful activities at home and community

· Maintain the daily operations of the home such as nutritious meal preparation based on diets, cleaning activities in compliance with health, safety and cleanliness standards.

· Assist in basic living activities of each individual including appearance, nutrition, recreation and socialization.

· Act as a role model to the individuals in areas such as appearance, emotional stability and social behavior.

· Encourage and work with individuals to progress towards their outcomes and record data

· Contribute as a member of the interdisciplinary process in:

o Assessing the desires, strengths and needs of individuals in residential services

o Developing objectives and outcomes

o Evaluating progress toward independence in outcomes

o Ensuring proper medication administration

· Complete and submit in a timely manner, required records and forms such as:

o Physical Examination Forms

o Medical Summary

o Dental Visit Summary

o Incident/Unusual Incident Report

o Individual financial ledgers

o Bi-Weekly petty cash forms and gas ledgers

· Ensure the proper and timely completion and maintenance of records including:

o Communication Logs

o Daily Progress Notes

o Data Collection Forms

o Medication Logs

o Outcome Data

· Communicate continuously with your Supervisor

· Secure the home and its’ contents by following standard procedure and protocols.

· Comply with the following safety policies and procedures:

· Must be an approved driver and must maintain that status.

Qualifications:

· High school diploma or G.E.D.

· Driver's license valid in their state of residence

· Excellent verbal and written communications skills.

· At least 1 year work history

Job Type: Full-time

Pay: $16.84 per hour
