Director I Care Consultant: Responsible for strategy and implementation of the Care Delivery Transformation programs.

Location: This role will support the CO, NV, and TX markets. The ideal candidate will reside within a 50 miles radius of a Pulsepoint in these markets.

Primary duties may include, but are not limited to:

• Builds culture and promotes Elevance Health’s diversity mission & vision.
• Ensures Care Consultants understand practice transformation model and success measures.
• Develops communication documents and training criteria related to program success measures.
• Develops documentation to support internal and external presentations as well as tracking and monitoring the value and success of program implementation.
• Collaborates with Care Delivery Transformation Directors and Program Directors related to program development and internal/external communication. Works with Regional Director to define state-level strategy that will roll up to regional plan, including caseload prioritization and staffing model that supports state-level Line of Business targets.
• Establishes relationships and meets regularly with Plan Presidents and Medical Directors.
• Works with Reporting Director to curate and distribute a set of standard reports to market contacts.
• Accountable for developing and maintaining Internal relationships as well as local market relationships. Maintains market level relationships, collaborates on a monthly basis with LOB, Quality and HCN leads (PE, VBS Contracting, etc.,)
• Participates in annual Care Delivery Transformation goal setting and talent calibration.
• Hires, trains, coaches, counsels and evaluates performance of direct reports. Develops Care Consultants to ensure they are driving cost & quality outcomes.

Minimum Requirements:

• Requires a BA/BS; 5 years of experience in external client facing program management, primary and specialty care transformation, quality improvement, and experience in a primary/ambulatory care setting; or any combination of education and experience, which would provide an equivalent background.
• Transformation Program Development and Implementation required
• Travel required.

Preferred Skills, Capabilities and Experiences:

• Broad range of experience with Commercial, Medicare and Medicaid programs preferred.
• MBA preferred.
• Project management experience preferred.
• Hands on People leader experience with diverse teams

For candidates working in person or remotely in the below locations, the salary* range for this specific position is $125,280 to $197,316.

Locations: Colorado and Nevada
