Overview:
Responsibilities of the Director of Social Service will include oversight of the Admissions Department and collaboration with the interdisciplinary team to ensure a smooth admission process and to support residents and families with ongoing adjustment to the facility.

  • Ensure that assessments and care plans are developed in a timely and meaningful manner. Provide residents and families with education and support regarding aging, illness, coping and loss.
  • The Director is the team leader in discharge planning and coordination of services needed for a successful transition back to the appropriate community setting. Follows up post-discharge of the CCRC residents within one week and then as needed.
  • Responsible for the administrative, operational and management needs of the social service department as reflected by the provision of quality resident care and the protection/advocacy of the range of resident rights in accordance with the facility’s philosophy, mission and goals
  • Prepare or revise policies, procedures, forms, plans, and programs to meet the psychosocial needs of residents in order for them to function at their highest practicable level and perform other activities as required or as dictated by position responsibilities.
  • Establishes and oversees the accomplishment of annual program goals and objectives for all psychosocial aspects of the Engaged Living program across all 4 services product lines
  • Schedule interdisciplinary family/resident discharge planning meeting as soon after admission as possible to address discharge potential, gather information required to increase the probability for a successful placement, and coordinate resources for a safe discharge.
  • Internally provides information to residents or families regarding Medicare/Medicaid and other financial assistance programs, referring to appropriate agencies or the Business Office as necessary without the use of an outside contractor.
  • Case Management/Discharge Planning: Manages a caseload and provide discharge planning services for all skilled including the short term population.
  • Refines existing Engaged Living social programs and develops strategies for new programming
  • Develops, implements and evaluates social based educational training and in-services for all care staff across all 4 service product lines including new regulatory requirements
  • Ensures effective communication among Engaged Living program leaders for positive social outcomes
  • Works with CentraState Pastoral Department and Healthy Aging Wellness Coach in the creation of collaborative programming with community agencies, medical organizations, and local congregations
  • Reviews and updates department policies and procedures and assists the management team to review
  • Addresses the ongoing psychosocial needs of residents across all 4 service product lines by providing a range of social/clinical services such as individual or group supports around adjustment or loss issues, assisting the Applewood care team and families of residents, and/or coordinating the services of the geriatric psychiatry consultation treatment team
  • Looks for ways to improve effectiveness by implementing new ideas and more efficient approaches. Identify efficiencies and opportunities that support CentraState growth of service, volume and/or meet demand
  • Leads Medical Director, Psych Director, clinical team and committee chairs to ensure appropriate medical staff involvement in resource management and optimizing patient’s length of stay
  • Participates on assigned senior services and hospital committees; promotes effective communication between departments, acting as a liaison to facilitate information sharing, collaborative problem solving and adequate provision of support services
  • Fosters and maintains collaborative relationships within Applewood, the Healthcare System; and with external agencies related to quality and performance initiatives as well as day to day operations issues that may arise.
Qualifications:

Education:

Master’s Social Work Required


Work Experience:

Long Term Care Social Work Experience Required


Licenses and Certifications:

Licensed Social Worker (LSW) Required


Required Knowledge and Skills:

Strong professional, organizational, and interpersonal skills to effectively relate with all members of the healthcare team
