The Developmental Disabilities Administration (DDA) is looking for a dynamic person to join our team as a Children’s Regional Transition Coordinator (CFC) Social and Health Program Consultant for Region 1. In this role, you will work in conjunction with program managers and quality assurance managers located at the headquarters office and across the state to implement a standardized process and supports for clients transitioning from one setting to another; serve as the children’s services specialist and provide the regional oversight and direction to reduce the number of former foster youth with developmental disabilities discharged into homelessness or inappropriately placed in hospitals; maximize the use of most cost-effective services and monitor enrolled clients between ages 16 and 21 years of age and eligible for Medicaid services for transition support needs; monitor and manage children transitions follow the statewide process and barriers are addressed to promote timely and stabile transitions; to serve as the regional subject matter expert and represent DDA in system meetings when Case Resource manager is not assigned, or client is working through DDA eligibility process. You will participate in DDA committees and workgroups related to the CFC program and report directly to the Transitional Care Regional Manager. This position can be located anywhere within the region 1 office.

Some of what you'll do:
  • Regional Representation on multidisciplinary meetings regarding children’s barriers to transition into other services.
  • Regional Representation on hospital transitions when additional management support is required or as directed by Transitional Manager, and/or Regional Administrator.
  • Substantial work as a regional subject matter expert on Children’s support services as related to transitions and transition time frames and processes for Children exiting DCYF services and supports.
  • Acts as liaison with the Department of Children, Youth, and Families for the region, leading staff direction with coordination activities and responses.
  • Responsible for regional planning, leading, developing, managing, and direction regional staff regarding program criteria for implementation of the statewide transition process and timelines for clients to transition timely and safely.
  • Substantial training of staff on statewide transition and tracking processes, and DCYF Service level agreements.
  • Ensure regional practices are adopting statewide policy and procedures, tracking and process improvement strategies with children transitioning into other settings.
  • Serve as the regional representation on stakeholder meetings and collaboration events to address system barriers to clients receiving timely and stable transitions.
  • Reviews and interprets the Administration’s mission, vision, and values. Interprets program policies and procedures, WAC & RCW, as well as other regulations to staff, families, clients, service providers, and other interested groups.
  • Manages regional programs as directed by the Transitional Manager. or the Regional Administrator.
  • Participate or implement root cause analysis of unsuccessful transition process to determine system improvements.
  • Understand and interpret the DSHS/DDA mission, program policies and procedures, WAC, and other regulations to staff, clients, families, service providers, and other interested groups.
  • Understand and follow RCWs, WACs, and policies and procedures pertaining to the duties of the position.
  • Listen, observe, communicate, and engage in problem-solving with clients/legal representatives, families, and providers to develop solutions that are client-centered.
What we're looking for:
  • Knowledge of the Developmental Disabilities RCWs, WAC’s, and programs/policies/procedures pertaining to duties of the position. Federal and State statutes pertaining to developmental disabilities programs.
  • Principles of legal process, witness interviewing, document collection and assessment, conflict resolution, negotiation, and litigation
  • Comprehend complex legal issues and prioritize a response.
  • Prepare professionally written reports, plans, and oral arguments.
  • Read, interpret, and implement policies, rules, and regulations of considerable complexity
  • The ability to compile data, extract information, and generate comprehensive reports.
  • Ability to self-direct and work independently with little supervision, organizing the work environment; prioritize, organize, and complete tasks/projects within deadlines
  • The Ability to interpret department/division/program policies and operating procedures to hearings personnel, staff, clients, families, and others. Analyze programs and policies.
Who should apply?

  • One year of experience as a Social and Health Program Consultant 3
Or,
  • Two years of experience as a Social Health Program Consultant 2 or equivalent
Or,
  • A Master’s degree in social work, a health or social science, public administration, or a related field and four years of professional experience in planning, administering, developing, or delivering social, financial, health, or chemical dependency treatment services programs
Or,
  • A Bachelor's degree in social work, health or social science, public administration, or a related field
And,
  • Five years of professional experience in planning, administering, developing, or delivering social, financial, health, or chemical dependency treatment services programs.
