Wage band range $21.00 to $33.00 per hour

Starting pay is typically between $25.00 to $28.00 per hour; however, it is ultimately determined by contract obligations, the scope of the position, the candidate's relevant experience, and internal equity. Position is eligible to test for additional bilingual pay incentive. This position will not be offered as a remote work style

Come join our team of supportive coworkers who coordinate the development and implementation of individualized service plans (Plan of Care), while providing direct services to children and their families to maximize the involvement of all persons and the implementation of plans, and facilitates the integration, coordination, and implementation of services. The Family Facilitator carries out the mission of Casa Pacifica and Camino a Casa program as it pertains to the program development of youth/families, the activities of youth/families, the clinical development of youth/families, and the general welfare of youth/families.

DUTIES AND RESPONSIBILITIES:

  • Along with other team members, assist in the ongoing development and evaluation of individualized support for clients and their families.
  • Provide leadership in the assessment, development, implementation, and evaluation of individual client and family plans and the Child and Family Team (CFT) Meetings.
  • Coordinate, schedule, and facilitate child and family team meetings in collaboration with other team members, referring party, and family for development of individualized Family Mission Statement, Plan of Care and Safety and Crisis Plan.
  • Assume oversight for the Plan of Care implementation and provide case management support as well as coordinate outside services and resources for client and families.
  • Assist in monitoring the Child and Family Plan expenses / budget and secure approval for overall budgeted plans as applicable.
  • Monitor and support the provision of unconditional care by reassessing and “redoing” individual plans as needed, ensuring that families have access to resources, and maintaining effective relationships with child and family team members.
  • Provide crisis management to families and child / family teams when plans disrupt and onsite consultation to others involved in plans.
  • Provide support, when family is in need.
  • Assume responsibility and document contact for ongoing interactions with referring agencies for every case, including contributing to the UR process.
  • Carry a case load averaging ten cases (or as needed to provide a minimum of 400-600 minutes of mental health support and 1250 total minutes of documented service per week as indicated).
  • Remain current and accurate with billing and meet target goals of 48-hour submission 80% of the time.
  • Submit required billing forms and progress reports on a timely basis.
  • Submit all time sensitive documentation (WFi-4, VCOS, intake paperwork, periodic paperwork, etc.) prior to or by deadlines.
  • Serve as role model and advocate for children and families in Wraparound.
  • Monitor and ensure effective linkages with community and agency service providers.
  • Participate in training activities as directed and in semi-monthly Family Facilitator Meetings, Camino a Casa meetings, as well as All Staff Meetings and Case Reviews.
  • Co-facilitate Matrix, Strengthening Families 10-14, and Familia Adelante groups as needed.
  • Conduct mental health assessments and develop treatment plan goals for children within the programs as assigned.

EDUCATION and/or EXPERIENCE: Master’s degree with two years’ experience working with seriously disturbed/at-risk children, adolescents, and their families. Experience engaging parents as partners and demonstrated ability to successfully work with families from diverse ethnic and cultural groups and well as maintain collaborative relationships with county partners. Experience in crisis management a must.

Must have a Master’s degree. Preference for MFT interns, ASW, LMFT, LCSW, Psy.D. or Ph.D. license or license eligible.

CREDENTIALS: Community First Aid and Cardiopulmonary Resuscitation (CPR) certification, preferred. Complete the Basic Wraparound Training as well as the Casa Pacifica New Employee Orientation Training. Additional trainings and certifications in Safe Environments, Life Space Crisis Intervention, and collaborative problem-solving a plus. Valid California Driver's License with excellent driving record. Use of personal car required, mileage reimbursement. Must have proof of insurance. Must maintain a positive driving record; reports tickets and driving points to Program Manager as they occur. Abides by Casa Pacifica travel policy. Must be agency approved driver.
