Caring Connections for Special Needs is looking to fill the Family Support Specialist – BHT position.

Overview of Role and Responsibilities:

Senior Clinicians are responsible for administering assessments, providing staff training and supervision, providing parent training and consultation, developing individualized treatment plans and monitoring client progress. They are a brand ambassador for the organization and strive to uphold the company’s values at all times.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Responsible for coordinating and providing services (with a focus on Family Support) with agencies and families to meet the needs of the patient according to the treatment plan
  • Serves as a secondary Assessor- completing assessments as scheduled
  • Maintains compliance with all company policies and procedures
  • Coordinates services according to treatment plan
  • Follows a set schedule to include all necessary agency/state requirements, Claimtrak notes & Relias learnings
  • Assists children and families to meet their needs in accordance with treatment plan
  • Provides direction and follow-up to families and individuals regarding their concerns and issues
  • Creates a safe and No Harm environment for the children and their families
  • Provides transportation to and from services as needed/required per treatment plan and the needs of the family
  • Drives company vehicle as needed or assigned
  • Provides information and referral assistance to families
  • Advocates for members with appropriate agencies
  • Complies with HIPAA guidelines
  • Follows Duty to Report and Therapeutic Boundaries
  • Actively communicates with site staff of any changes
  • Documents all services provided via Claimtrak within 24 hours of services
  • Reports all incidents within 24 hours to Supervisor and complete appropriate forms/documentations as advised by CCSN P&P
  • Attends mandatory Supervision (required to make up any missed Supervision ASAP)
  • Communicates with Site Supervisor hours worked including all anticipated overtime
  • Submits daily worksheets within 24 hours to Site Supervisor
  • Submits timecard according to CCSN guidelines and policies
  • Maintains professionalism with all agencies in our community to be a key asset in coordinating outside resources
  • Other duties as assigned

Requirements

Education Requirements:

  • Has a master's degree or bachelor's degree in a field related to behavioral health;
  • Is a registered nurse;
  • Is a physician assistant who is not working as a medical practitioner;
  • Has a bachelor's degree and at least one year of full-time behavioral health work experience;
  • Has an associate's degree and at least two years of full-time behavioral health work experience;
  • Has a high school diploma or high school equivalency diploma and:
  • 18 credit hours of post-high school education in a field related to behavioral health completed no more than four years before the date the individual begins providing behavioral health services and two years of full-time behavioral health work experience; or
  • Four years of full-time behavioral health work experience; or
  • Is licensed as a practical nurse, according to A.R.S. Title 32, Chapter 15, with at least two years of full-time behavioral health work experience.

Work Experience:

  • Must be a state-certified Family Support Partner
  • Excellent verbal and written communication skills, including ability to effectively communicate with internal and external customers
  • Excellent computer proficiency (MS Office – Word, Excel and Outlook)
  • Must be able to work under pressure and meet deadlines; while maintaining a positive attitude and providing exemplary customer service
  • Ability to work independently and carry out assignments to completion within parameters of instructions given, prescribed routines and standard accepted practices
  • Applicant must be 18+ years of age or older
  • Valid Arizona Driver’s License
  • Clean driving record
  • Must maintain current/valid/restriction free Level 1 Fingerprint Clearance Card
  • Must have a current certification in Child/Infant CPR/First Aid
  • Reliable transportation to get to and from the assigned work setting

o Individualized Assessment & Service Planning in the office or home

o Individual & Group Respite including office, home, and community-based events

o Individual & Group Skills Training focusing on relationships, problem solving, anger management, social skills, boundaries, coping skills, positive behavior change, communications, etc.

o Family Support Services to integrate skills, assist with challenges, and provide education/referrals

o Coordination with other providers including the Department of Child Services, local schools, community agencies, juvenile justice, etc

o Participation in Child & Family Team (CFT) and Individualized Education Program (IEP) Meetings

o Transportation to and/or from services, if applicable

Job Type: Full-time

Pay: $17.00 - $20.00 per hour
