LOOKING FOR A MOTIVATED, SENSITIVE, EMPATHETIC INDIVIDUAL TO PERFORM CASE MANAGEMENT AND PROVIDE DIRECT SUPPORT TO INDIVIDUALS WITH INTELLECTUAL DISABILITIES WITHIN A CREATIVE AND DYNAMIC ENVIRONMENT


Essential Duties and Responsibilities

  • Maintains a caseload of individuals with intellectual disabilities
  • Attends meetings with individuals' treatment teams both within the organization and externally.
  • Complete timely documentation related to services utilizing an online EHR system.
  • Participate in establishing new goals for individuals.
  • Conduct groups/activities in a group setting for individuals with Intellectual Disabilities at Gateway and in the community.
  • Supervise individuals during field trips to work sites.
  • Responsible for creating group activities and assisting with lesson plans that incorporate the individuals goals and staff actions.
  • Train and supervise individuals in all aspects of volunteer work, including safe work practices in the building and in the community.
  • Serve as transporter/matron for individuals being transported on trips within the community.

Education and/or Experience

Associates or Bachelor's degree (B. A.) in Health or Human Services or related field from accredited College or university

Computer Skills

To perform this job successfully, an individual will be required to document services utilizing an online Electronic Health Records system (with training).
