Health Appointment Tech – Adult Group Home – 40 hrs/wk

Shift: Monday - Friday; 8:00A - 4:00P

Qualifications:
- Valid NYS Driver’s License required
- Previous OPWDD Experience required
- AMAP Certification, including g-tube certification required
- CPR/AED preferred
Key Responsibilities include, but not limited to:
- Accompany residents on medical appointments.
- Ensure physician completely fills out medical referral form.
- Enter appointment notes in residents’ medical charts.
- Assist in maintaining medical records.
- Provide assistance and instruction in basic activities of daily living to each consumer.
