Home Health Aide Job at Grand View Health
Schedule: Full Time, Days - Mon-Fri, 8am-4:30pm. Weekend 1 out of 4.
Responsibilities
Under the direction of the professional nurse, the Home Health Aide assists the patient in the home setting with personal care activities and incidental duties which the patient requires. The Home Health Aide ensures assessment of care is appropriate to the age of patients served: adolescents, adults and geriatrics.

- Providing personal care to adolescent, adult and geriatric patients according to established standards in order to maximize patient comfort.
- Assessing the patient's temperature, pulse, respirations and blood pressure to monitor his/her condition.
- Changing and making bed to maintain cleanliness of patient bed.
- Assisting patients with prescribed exercises.
- Assisting patient with application of TED/JOBST stockings.
- Observing patient's condition and report changes in condition to the Registered Nurse or Supervisor.
- Assisting with patient transfers, ambulation and prescribed exercises according to established procedures.
- Documentation of care provided and other pertinent observations in the clinical record to promote continuity of care.
Grand View Health's Home Care department serves Bucks and Montgomery communities. GVH Home Care strives for excellency by assisting patients with their health needs in the comfort of their home. The Home Care staff is expected to commute within a 15-mile radius of Grand View Hospital. Gas mileage will be reimbursed.
Qualifications
EDUCATION AND EXPERIENCE:- High school diploma/GED minimum.
- CNA or must have successful completion of formal training program with required 75 hours of training and 16 hours of supervised practical or clinical training.
LICENSES AND CERTIFICATIONS:
- Basic Life Support (BLS) upon hire required.
- Driver's license – PA, upon hire, required.
- Automobile insurance/registration upon hire required.
