CLINICAL MANAGER

The Clinical Care Manager is responsible for providing support to the Branch Director in obtaining qualified hospice personnel; is responsible for the provision of hospice services, directly, and the delegation to and coordination of hospice personnel evaluations and day to day operations. The Clinical Care Manger supports the objectives for hospice services that meet and promote the standards of quality of care and contribute to the total organization and philosophy of the hospice program including participating in the Quality Assessment and Performance Improvement Program. The incumbent is responsible for working with the Branch Director in establishing standards of care to comply with federal and state regulations and guidelines.

Responsibilities:

  • Coordinates patient care. Coordinates and integrates care provided by Agency staff, including contractual providers.
  • Complete workflow tasks, Administrative Tasks, and other assignments to the Clinical Care Manager Responsible Position in the Agency’s electronic medical record.
  • Receives and enters orders from all relevant physicians, Nurse Practitioners, and/or Physician Assistants involved into the plan of care and ensures the orders are approved by the responsible physician.
  • Ensures effective communication with all physicians involved in a patient’s plan of care and other health care practitioners (as appropriate).
  • Assist in the entry of patient referrals, as needed.
  • Follows up on pending referrals to ensure timely patient admission in collaboration with the Intake Coordinator.
  • Prepares for and participates in case conference in collaboration with other healthcare team members and participates in decision-making regarding patient recertification/discharge.
  • Reviews patient occurrence reports and takes appropriate action.
  • Reviews patient infection control reports to ensure appropriate action has been taken and items are reported to the QAPI committee.
  • Reviews On Call coordination notes and take appropriate action.
  • Assures that patient needs are continually assessed. Assures reassessment of the patient’s needs is performed by the appropriate healthcare professional when there is a significant health status change in the patient’s condition, at the physician’s request, and after hospital discharge.
  • Assures the development, implementation, and updates of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s).
  • Ensures provision of services as ordered by the physician as indicated in the plan of care, including patient and caregiver education and patient, caregiver, and family counseling. Prepares clinical notes as appropriate.
  • Assumes responsibility for continuity, quality, and safety of services delivered in compliance with State and federal regulations.
  • Monitors systems and identifies problem areas to Branch Director both verbally and through written reports.
  • Meets mandatory continuing education requirements of the Agency and licensing board.
  • Promotes and educates regarding the concepts of infection control and standard precautions in coordinating or performing patient care activities to prevent contamination and transmission of disease.
  • Uses effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
  • Participates in Agency-sponsored in-service training.
Qualifications:
Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing. May be required to complete HHS’ Computer Based Training for appropriate category(ies) of license.

Licensure: Current license as a registered nurse (RN) in state of residence and/or in accordance with the Nurse Licensure Compact (NLC), current Driver’s License.

Experience: Minimum 2 years’ experience as a Registered Nurse in a clinical care setting, and one year homecare experience. Management experience preferred.

Skills: Nursing skills as defined as generally accepted standards of practice. Excellent interpersonal skills and the ability to communicate effectively with all segments of the staff. Demonstrates proven decision-making skills. Proof of current CPR.

Transportation: Reliable transportation. Valid and current auto liability insurance.
