Our Homeless Department focuses on housing individuals and families in our region through extensive community outreach and collaboration. The Housing Team provides assistance and access to housing and treatment for the homeless through a variety of Housing and Urban Development (HUD) and Substance Abuse Mental Health Services Administration (SAMHSA) funded programs. The Housing Development Assistant seeks out and supports homeless individuals with the goal of accessing housing and treatment for mental illness and/or substance misuse. They also assist in the writing and renewal of HUD and SAMHSA grants. This is a full time, 35 hour per week position.

Major Functional Areas and Description of Duties

  • Perform Projects for Assistance in Transition from Homelessness (PATH) outreach to the homeless within Western Rockingham County to assist and engage those who are homeless towards finding permanent housing, as well as treatment for mental illness and/or substance misuse;
  • Refer individuals for clinical screening, diagnostics, and case management services;
  • Participate within the Local Services Delivery Area (LSDA) and foster cooperation and collaboration between various service providers, as well as follow through on referrals made to CLM in regards to possible homeless individuals;
  • Attend and participate in the Homeless Management Information
    System (HMIS) Data Committee, Trainings and Advisory Meetings, Housing Action New Hampshire Workgroup meetings, the Balance of State Continuum of Care (BOSCOC) meetings and various subcommittee meetings as required;
  • Assist in the development of Notice of Funding Availability (NOFA) documentation and HMIS Annual Performance Reports;
  • Assist individuals with applying for Medicaid and Social Security benefits;
  • Assist with working with landlord’s, gaining new landlord’s as well as search for apartment’s and fielding calls;
  • Assist with other housing programs such as the Bridge Program, New Hampshire Housing, etc.;
  • Work collaboratively with other treatment team members, natural supports, and external providers to ensure a wrap-around approach is provided for continuity of client care;
  • Function professionally and in a manner that protects the integrity, confidentiality and rights of all patients;
  • Adhere to and execute all required services, timely documentation and HMIS data collection in accordance with agency policies, federal and state standards;
  • Maintain professional memberships, licenses, accreditation, certifications, etc.;
  • Represent CLM in a professional and appropriate manner in all settings;
  • Perform all other duties as assigned by supervisor.

Qualifications and Skills

  • Bachelor’s degree preferred however an Associate’s degree along with a minimum of two years of direct service in a related field may qualify;
  • Understanding of homelessness and prior experience working with mental illness and or substance misuse disorders.
  • Ability to exercise sound clinical judgment in routine, urgent, and emergent clinical situations;
  • Proficient computer skills, specifically with all Microsoft office applications;
  • Ability to document effectively and efficiently in course of clinical work;
  • Ability to navigate an electronic health record;
  • Ability and desire to work as part of a clinical team while also able to be self-directed and motivated with minimal supervision;
  • Ability to acquire and effectively practice new clinical skills as needed according to evolving agency needs/services;
  • Effective communication skills, organizational skills, and ability to multi-task.

Physical Demands and Working Conditions

Must be able to walk in any terrain within Western Rockingham County. Must have weather appropriate attire for being outside, especially in inclement weather. Must be able to sit for 60 minute intervals and be able to sit at a computer for data entry. A majority of the work will be visiting clients in the community and also in their homes. The ability to sit for extended periods of time is required. Reliable transportation, a clean driving record, and proof of auto insurance coverage is required.

All candidates for employment who are given an offer of employment must either provide proof of being fully vaccinated through CLM's COVID-19 Vaccination Status Form or complete the primary vaccination series for COVID-19 by their first date of employment unless they request an exemption and CLM grants the request.
