Family Health Centers of San Diego (FHCSD) is looking for a Housing Navigator to meet with clients to determine eligibility, complete all assessments and develop Individualized Service Plans in partnership with the client, complete all record keeping requirements to allow the accurate collection of measurable outcome data and complete data entry into required systems of care, provide warm handoffs and follow up for all cross-referrals made, and work with the client to obtain the necessary documentation and resources to ensure they are ready for housing placement. HNs will also participate in regularly-scheduled training opportunities provided by FHCSD, to ensure the best possible outcome for their clients.
The Homeless Emergency Aid Program (HEAP) will be used for the provisions of Housing Navigation services.
Responsibilities:
  • Collaborate with multidisciplinary care team in coordinating a wide array of outreach, health care, case management, patient engagement functions in supporting and connecting patients of the Homeless Population with FHCSD services.
  • Develop relationships with this special population by use of best practices techniques involving Motivational interviewing, Trauma Informed Care, Critical Time Intervention, Harm Reduction, and Housing First modalities.
  • Provide direct and indirect patient services to support linkage and maintenance in care to include internal completion of internal and external referrals for the assigned programs.
  • Resolve any barriers related to necessary resources in supporting this special population.
  • As the intermediary, assist in communicating and clarifying information to access of needed resources.
  • Provide relative information on specific services and resources available that can be accessed and utilized.
  • Assist client in navigating through the various services in connecting to the most appropriate and viable resources.
  • Interact with all levels of the organization, in developing and sustaining relationships to service clients’ needs and resources.
  • May be involved with patient quality improvement projects to include improving retention in connected resources and services.
  • Completes all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards. Assists in preparing reports as required.-
  • Conducts initial and on-going assessment of client's health and/or support service needs. Sets level of client need.
  • Creates formalized case plans and goals with clients. Develops a written care-plan with the client, identifying problems and needs. Plan includes areas assessed, intended interventions, and expected results in measurable terms, with short and long term goals. Updates plan as client's needs change.
  • Perform other duties as assigned.
Requirements:
  • Bachelor’s degree in social work, public health, science, or related degree preferred.
  • High School diploma (or GED equivalent) required.
  • 2 years of experience with health education or working in the medical/healthcare related field.
  • Knowledge of a wide array of services and resources related to outreach/patient engagement, healthcare, case management, and/or care coordination.
  • Knowledge of the community resources, health and social services systems in San Diego County, and possess skill to establish working relationships with community partners.

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors – we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
