POSITION SUMMARY

The Hospice Clinical Team Manager is responsible for the overall management, coordination and supervision of clinical services for the Hospice Team

ESSENTIAL DUTIES AND RESPONSIBILITIES

The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned. .

  • Assure patient care to be of the highest quality by providing guidance, direction and oversight for the clinical team
  • Ensure appropriate staffing of patients through coordination with scheduler. Review referrals, update and communicate any changes as needed
  • Coordinate coverage for on-call scheduling
  • Collaborate with the Hospice Director to facilitate and coordinate hospice services
  • Ensure staff meets quality, safety and competency requirements through new staff orientation and on going training and development.
  • Review clinical documentation for all staff to assess for completion, presence of essential and required information, timeliness of entries, and level of comprehensiveness necessary to meet productivity, practice and regulatory standards.
  • Timely and accurate review of admission/recertification documentation, orders and plans of care

Qualifications:

  • Graduate of an accredited nursing school
  • Current license to practice as a Registered Nurse in the state of Massachusetts
  • A minimum of three (3) years of hospice experience
  • At least five (5) years of nursing experience; one year in supervision, education or nursing service administration
  • Management experience, preferably in a Hospice or Home Care setting
  • Demonstrated commitment in the Hospice philosophy of care
