Starting Pay Range –

Bachelor Degree - $19.30 - $21.22/hour

Master Degree - $22.15 - $25.04/hour

Position Description:

Case Managers work with consumers who have mental illness. They are part of a multi-service treatment team. Case Managers are often the point person for the treatment team and help facilitate and coordinate services within and without CRMHS. Case Managers offer education about mental illness, assist consumers with achieving their goals, and help with skill building to promote independence and recovery. Case Managers work with individuals, family members, significant members of social and support networks and lead a variety of psycho-educational groups. Case Managers may work with their clients in the clinic, in the home, and in the community.

This position is a housing case manager position and involves a high level of coordination with community partners, such as the VHA and Council for the Homeless. Case manager will manage the referral process for the housing options through the VHA and Key properties, such as: Central Park Place (SRO), Rhododendron Place, Teeny Creek.

The Housing Specialist will have two primary scopes of work: 1, home and community-based supports to eligible individuals- which includes coaching, advocacy, information and referral, linking and coordinating, and ongoing supports; and 2, identifying housing opportunities in the community by doing outreach to private market, and scattered-site landlords.

Essential Job Functions:

The Case Manager works collaboratively with each client to develop a unique treatment plan based on the individual’s recovery goals. The treatment plan addresses the following life domains: Mental Health; Alcohol/Drug Issues; Activities of Daily Living; Work/School; Family/Natural Support; Income/Finances; Social/Friends/Fun; Legal: Medical and Dental; Housing/Shelter; Cultural/Spiritual; Other.

  • Case Manager will work on site at Rhododendron place approximately 2 days a week.
  • This position will support clients already in CRMHS housing and housing retention.
  • Maintain all required paperwork/charting requirements as established by agency contracts.

· Achieve and maintain agency standard productivity requirements.

· Tennant-Landlord Mediation; Preparing landlords for high acuity clients entering their units.

· Provide ongoing tenancy supports as needed (and for as long as the individual remains eligible) so that the individual can remain stably housed.

· Conducting functional needs assessment to identify participant tenancy preferences.

· Developing individualized community integration plans based on the functional needs assessment.

· Working with participants to develop short and long-term measurable goals.

· Providing service-planning support and participate in person-centered plan meetings.

· Coordinating and linking participants to community support services i.e. primary care provider, behavioral health services, etc., including those supports designed to obtain and maintain independent living.

· Providing entitlement assistance (including obtaining documentation, navigating and monitoring the application process, and coordinating with the entitlement agency).

· Providing supports to assist participants in the development of independent living skills.

· Coordinating with participants to review, update and modify their housing support and crisis plan.

· Connect participant to resources that will assist in being a good tenant.

· Working with private market, scattered-site landlords, grant funded opportunities to grow and foster opportunities for housing placement

Non-essential Job Functions:

- Supports agency mission, goals, and objectives.

- Attend staff and planning meetings as scheduled.

- Maintain communication and cooperative working relationships both within the agency and community at large, while maintaining a high degree of confidentiality.

- Other duties as assigned.

Education, Licensing and Experience:

· Bachelor degree in mental Health, Social Work, Psychology, or Human Services

· Approximately 1-2 years of experience in the social service field with mental illness or chemical dependency preferred.

· Approximately 1-2 years of experience working in the housing realm preferred
