YOUR DUTIES AT A GLANCE
In this position you will Housing Support Services to Medicaid service consumers in need of necessary supports to obtain and reside in an in-home setting. Supports are tailored to the goal of maintaining a consumer’s personal health and welfare. Supports are offered through consultation with a consumer’s Medicaid Case Manager or in working directly with a consumer. Housing Support Services include but are not limited to conducting a tenant screening and housing assessment, developing an individualized housing support plan, developing a housing support crisis plan, identifying and sharing resources, assisting with application processes and move details, offering education and training, coaching and providing assistance, and advocating.
Major duties include:
Assessment and Determination
  • Evaluates service needs and develops individualized Housing Support Services assistance plan based on factors such as psychosocial, cultural, financial, and medical conditions.
  • Provides Housing Support Services for individuals either through consultation with the consumer’s Medicaid Case Manager or in working directly with the individual.
  • Provides counseling related to crises in individual's condition, and helps individuals and families make decisions regarding issues surrounding placement to maximize the functioning of the client's natural support system.
  • Facilitates individual’s understanding of housing tasks and the completion of tasks. Assists individual by helping to problem-solve and provide tools applicable to Housing Support Services.
  • Screens for assistance through a referral from a Medicaid Case Manager and will follow up with family members, providers, physicians for additional information and confirmation, as needed.
  • Assesses for the need of the service based on the following information and criteria: functioning capacity with behavior, coping abilities, injury disability, condition, and needs; and support systems available for service delivery.
  • Determines acceptance or denial of the referral and communicates outcome to the Medicaid Case Manager and/or individual.
WORKING CONDITIONS
This position will usually work a standard workweek. There may be some irregular hours due to consumer needs. Day travel will be regularly required within the service area; some overnight travel may be required for training. Travel may be in inclement weather. This position will require the ability to work on tasks simultaneously, sometimes with short time frames and will require regular contact with a variety of people, including housing related partners; ODHS and other state employees; employees of local governments; partners and consumer advocates; long-term services and health care providers, and employees of other state governments and of the federal government. Upon management approval, this position may have the option to telecommute or a hybrid work option.
THIS IS WHAT YOU NEED TO QUALIFY:
Six months of experience providing services to persons who are elderly or disabled; AND training and experience which shows knowledge of at least four (4) of the areas listed:
1) Medical and psycho-social problems of the elderly and disabled.
2) Normal and abnormal human development and behavior.
3) Appropriate intervention methods for the elderly and disabled.
4) Medical terminology and procedures.
5) Implications of illness or injury upon clients and families.
6) State and Federal rules about long-term care, medical services, and clients' rights.
7) Policies and procedures about agency programs.
8) Available community resources.
9) Crisis intervention, counseling, being an advocate, community relationships, and/or referral methods.

ESSENTIAL ATTRIBUTES:
  • Personal computer knowledge and proven competency in producing a variety of documents
  • Experience communication orally and in writing with a variety of people
  • Experience explaining policy and decision orally and in writing
  • Experience assessing physical and cognitive need
