About the Opportunity

This position provides data management and collection support to the WVU Positive Health Clinic. Incumbent is responsible for medical case management services, coordination of patient care, referrals, and coordinated and comprehensive patient care for HIV/AIDS patients in the Physician Office Center (POC) and Glen Dale Satellite Clinic; provides assessment and nursing intervention under medical supervision.

This position serves as a resource contact and independently uses professional concepts and techniques, as well as organizational policies to solve a wide range of moderate to complex problems in imaginative and practical ways. Depth skills, knowledge, and interpretive abilities, as well as some creativity, are required to successfully perform duties. Work effort often addresses non-routine matters within established protocol, guidelines, and/or policies. Work effort has significant impact on client services and operations, producing moderate to major organizational, legal and financial results and/or liabilities. Position duties may require oversight of projects including coordination of work by others providing support of basic objectives.

What you’ll do:

  • Responsible for collection of patient data for entry into software programs for data management and reporting purposes.
  • Serves as primary contact for all patient telephone calls, responsible for prescription refills, triaging phone calls, scheduling / rescheduling / no shows, reinforces educational messages delivered by clinical staff and determines potential barriers for HIV/AIDS patients, and provides coordinated and comprehensive patient care including initial assessment and nursing intervention under direct medical direction.
  • Responsible for coordination of the clinic flow and clinic staff to ensure that each patient visit is complete – involving processing of patient weight, height, temperature, pulse, blood pressure, PHQ 9, vaccinations, visit with the physician, psychiatrist, Care Navigator/Medical Case Manager, and incoming referrals for linkage to care.
  • Responsible for preparing and presenting patients in the Weekly Patient Care Conference as requested. The purpose of this conference is to discuss all of the clinical aspects of each patient’s visit. This typically involves all patients seen in the clinic during the prior week and any hospital admissions or patient emergencies. Preparation involves monitoring laboratory results, checking all doctors’ notes, updating each patient’s encounter form, making sure the patient has a follow up appointment, and documenting recommended actions were appropriate.
  • Responsible for coordination and travel of the clinical care team to the Northern Panhandle satellite clinic. Responsible for the processing of patients, vaccinations, adherence counseling, monitoring laboratory results, coordinating referrals, and scheduling.
  • Maintains personal skill levels within specific areas of expertise by attending appropriate educational courses, programs, and conferences related to HIV. Maintains CE requirements for Licensure of a Medical Assistant in the State of West Virginia.
Qualifications
  • Completion of a medical assistant program;
  • Eight (8) years of experience in clinical patient care; Or an equivalent combination of education and experience.
  • Skilled at working as a team member
  • Should possess good written/oral communication, and have knowledge of basic computer applications using Word and Excel.
  • Excellent customer service skills and the ability to communicate effectively with patients and families; pharmaceutical company personnel; regulatory and monitoring agency personnel; and, internal staff members. Communicates with physicians, patients, students and employees of WVU, WVU Medicine, MBRCC and UHA.
  • Working knowledge of appropriate patient assessment techniques.
  • Knowledge of the information and techniques needed for diagnosis and treatment. This includes symptoms, treatment alternatives, drug properties and interactions, and preventive health-care measures.
  • Knowledge of appropriate bio-specimen collection and processing techniques.
  • Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures or governmental regulations.
  • Ability to write reports, business correspondence, and assist with procedure manuals.
  • Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
  • Ability to develop constructive and cooperative working relationships with others and maintain them over time.
  • Ability to analyze information and evaluate results to choose the best solution, solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form.
  • Ability to develop specific goals and plans, to prioritize, organize and accomplish tasks independently and through others.
