Responsibilities:

The Intake Coordinator position offers an exciting opportunity to lead the Sandstone customer experience, using creativity, communication, and teamwork to create a smooth landing and sturdy foundation for an exceptional patient stay. Candidates for this position are strong team collaborators and possess the ability to walk in the shoes of the customer, anticipating client needs and using proven customer service techniques and a team-centered approach to elevate patient satisfaction beyond industry standards. Sandstone Intake Coordinators, frequently use strong organization, communication, time management, and people skills to galvanize patients, families, and departments, delivering a strong start to ensure an equally strong finish.   • Accurately translates census and isolations onto bedboard
  • Communicates accurate bed availability, discharges, and room changes D
  • Develops room change plan, communicating and coordinating with various departments
  • Ensures all rooms and equipment are clean and in good repair
  • Completes admission agreement with patient or appointed representative,
  • Orientates new patients and families to building and building processes
  • Acts as customer service advocate during the intake process
  • Documents and communicates grievances and complaints discovered during the intake process per buildings specific and or QAPI policy.
  • Routinely monitors incoming patient list, coordinating and communicating with Centralized Admissions
  • Conducts facility tours
  • Other duties as assigned

Qualifications

  • High School diploma
  • SNF/healthcare experience is preferred
  • Excellent customer service and strong communication skills
  • Must be able to relate positively and favorably to residents, families, and government regulators and, to work cooperatively with other associates at all levels.
  • Must pass a criminal background check, drug test, and reference inquiry.
