Your Job as an Intake Coordinator:

As an Intake Coordinator you are responsible for completion of initial intake assessments, the admission of patients into the facility and maintaining communication between the organization,referral source, and family.

Essential Functions as an Intake Coordinator:

• Gathering and assessing patient clinical information for the purpose of making appropriate admission decisions
• Review prospective admissions against approved admission criteria, policies and procedures
• Initiate contact to gather required clinical and demographic data from patient and other sources
• Respond promptly to inquiry calls
• Conduct pre-admission clinical assessments to assist the physician in determining the appropriate level of care
• Refer inquiries to other agencies and community resources when not appropriate for facility assistance or admission
• Maintaining all the documentation involved with the admissions process

Available Shifts:

• Monday-Friday 4:00pm-12:00am
• Friday-Tuesday 2:00pm-10:00pm

Education/Experience Requirements:

• Bachelor's Degree in Psychology, Social Work, Counseling or Human Services field required
• Minimum one year experience with completing clinical/behavioral assessments

Job Types: Full-time, Part-time

Pay: From $16.00 per hour
