Overview

Title of Position: Intake Coordinator (Detox/Substance Abuse Center)

General Duties

The Patient Intake Coordinator plays an instrumental role in displaying compassion and communicating effectively with individuals currently experiencing substance dependence. They provide guidance and support incoming patients through the initial process of admission such as conducting phone intakes to collect relevant health information, establish individuals’ goals for treatment and determine eligibility possible treatment services.

Responsibilities

  • Clinical review of applications for services and supporting documents
  • Outreach and engage with potential patients via email, SMS, phone and live chat to schedule intake calls
  • Maintain clear and effective communication with callers, establish a positive rapport, and make accurate assessment of caller’s inquiry
  • Communication with and education of families and individuals seeking services on the eligibility process and the evidence necessary for determining eligibility
  • Assists families and individuals with obtaining required documents
  • Verify insurance coverage and schedule first appointments based on provider availability
  • Register patients and document relevant clinical information within our EHR
  • Coordinate with patient’s new care team to ensure a seamless transition into treatment
  • May develop, arrange, and present training requested by community partners to create broader understanding of services, waivers, and the process by which one obtains services
  • Determination of the adequacy of submitted documentation for submission to psychologist
  • Provide patients with information and referrals for community resources that may support their treatment.
  • Participate in program development efforts and provide input to optimize the enrollment process.
  • Other duties that may be assigned from time to time;

Qualifications

  • Minimum 1-year administrative experience interacting with patients in a fast-paced clinic, hospital, or community health center.
  • Previous experience working with individuals experiencing substance use dependency preferred
  • Strong verbal and written communication skills.
  • Ability to quickly adapt to a demanding, evolving, and fast-paced environment.
  • Proficient in using modern technology systems such as Google Sheets, Drive, Calendar
  • High school diploma or equivalent required with college degree
