Intensive Case Manager

Provide intensive case management services to Camden County clients. Engage with clients upon admission, throughout treatment, and upon discharge. The intensive case managers will carry a caseload of 25-50 active clients, and each will serve as a single point of contact in order to achieve continuity of care. Require comprehensive assessments; development of recovery plans that incorporate the clients' needs, priorities and preferences, as well as goals and action steps; and the coordination of various services aimed at addressing the identified needs and goals.

Responsibilities:

  • Conducts assessments and service planning on an ongoing basis.
  • Develops recovery plans in partnership with the client.
  • Coordinates care and services with all members of the inter-agency team and across multiple health and social service systems.
  • Links clients with professional services such as treatment, transitions between care levels, mental health and medical treatment, and other health providers.
  • Helps clients to obtain entitlements for which they qualify.
  • Provides referrals to legal, court, housing, employment, and recovery assistance.
  • Connects clients to support services based on the needs identified in their recovery plan.
  • Links clients to services geared toward safety, self-sufficiency, and positive family relationships.
  • Helps clients connect to legal resources as well as assisting them with ce1tain processes.
  • Connects clients to Certified Peer Recovery Specialists and recovery centers.
  • Supports clients in level of care transitions.
  • Provides client advocacy in multiple areas and with multiple systems.
  • Conducts active and assertive outreach to clients
  • Supports clients in meeting their self-directed recovery goals
  • Performs other related responsibilities and duties as required.

Education/Experience:

  • Bachelor's degree in Social Work, Human Services, or Addictions or related field, is required.
  • Three years’ experience as drug and alcohol case management specialist is required.
  • Microsoft Office (Word, Outlook) is required.
  • Knowledge of the basic principles, philosophies, practices, and procedures involved at drug treatment facilities is required.
  • Knowledge of substance abuse and co-occurring disorders is required.
  • Must be able to develop a therapeutic relationship with patients while maintaining therapeutic boundaries.
  • Excellent organization, written and oral communication skills is required.
  • Must have the ability to take initiative and use innovative techniques in preparing proposals.
  • Must have ability to establish and maintain effective working relationships with patients, other members of staff, outside agencies, institutions, and the general public.
  • Must possess excellent time management skills to plan, prioritize, and coordinate multiple projects.
