Our Health Center Locations: Brick, Freehold, Lakewood, Little Egg Harbor, Manahawkin, Manchester and Toms River; with our school-based Wellness Programs located within the Clifton Ave Grade School, Lakewood; and Lakewood High School.

OHI services include family and internal medicine, pediatrics, behavioral health, nutrition, OB/GYN women’s health, dental, family planning, specialty care, pharmacies, PrEP, and a STI clinic.

Position Summary

Under the direction of the VPSDOH, the Licensed Clinical Social Worker (LCSW) is responsible for the implementation of the Social Work Department Strategic Plan. The LCSW will work in an environment that supports clinical excellence and competency through multi-disciplinary teamwork and collaboration. The LCSW will develop a plan for social work intervention that is consistent and congruent with the medical treatment, considering the patient’s overall psychosocial status, physical and emotional needs. The LCSW will establish and maintain a therapeutic relationship with the patient, family and/or significant others and with their participation, formulates treatment goals that address identified needs or problems and a plan of action for achievement of these goals.

Responsibilities

  • Provides social services and assistance to improve the physical, social and psychological functioning of patients and their families.
  • Identifies psychosocial status, economic and physical needs of patients; assesses and reassesses patient’s support systems, available community resources and other factors to plan, develop, and implement an appropriate plan of care.
  • Provides psychosocial assessments, intervention, and care plan to patients of the Health Center and assists them and their families in coping with their illness, diagnosis, and/or life situation and meet the continuing health care needs.
  • Identifies emotional or behavioral issues and diagnose disorders in accordance with ICD-10 codes, using information obtained from screening tools, interviews, records and reference materials.
  • Counsel individuals and groups (if applicable) in order to modify behavior to improve personal, social and vocational adjustment.
  • Develop and implement individual treatment plans specifying type, frequency and duration of therapy.
  • Provide clinical services via tele-health consistent with established guidelines for best practice interventions.
  • Maintain working and functional telehealth equipment.
  • Provide a professional and confidential working environment for telehealth clients.
  • Manage organizational communications daily (i.e. stay current with emails document daily tasks as requested.)
  • Attend required meetings (by video/phone) such as supervision, team, and mandatory trainings.
  • Operates as an advocate with all community populations utilizing skills that respect issues of cultural and ethnic diversity and equity for every patient and family.
  • Coordinates services to Health Center, community agencies and other professionals; maintains working knowledge of and liaison with community agencies; participates in area activities related to health and social services.
  • Assumes responsibility for their own continued professional development; keeps current with social work and health care developments and seeks to increase further enhancement of job related knowledge.
  • Collaborates with the health care team and involves the patient/family in the development and implementation of plans.
  • Facilitates patient care across a multidisciplinary care continuum which includes interacting with Clinic Providers, and other health care providers to facilitate movement of the patient through the system and arrangement of an appropriate plan of care.
  • Serving as a patient advocate, maintains a working knowledge of relevant medical/legal issues that have an impact on patient care, e.g., advance directives, child and elder abuse.
  • Utilizing the EMR system, maintains records and statistics in accordance with department policies.
  • Attends and participates in staff committees, departmental clinical collaboratives and other administrative meetings.
  • Contributes to the efficiency and effectiveness of the Health Center’s service to patients by offering suggestions and directing or participating as an active member of the health care team.
  • Works collaboratively with all members of the Senior Management and Management Teams to resolve departmental and organizational challenges expeditiously, economically, professionally and with minimum disruption to the practice.
  • Constantly strives to achieve excellence in day-to-day operations of respective department.
  • Completes all mandatory in-services and attends required meetings.
  • Adheres to Corporate Compliance policies.
  • Maintains a passing monthly scorecard threshold at 80%
  • Other Duties as Assigned

Education/Experience/Licensure

  • Bachelor’s Degree in related field.
  • Master’s Degree in social work is required.
  • Three to five years as social worker in a community health care environment, medical center or other related field.
  • Active State of New Jersey Licensed Clinical Social Worker.
  • Proficiency in Microsoft Office 365 is required.
