$750 Sign-on Bonus

SUMMARY A person is this position is responsible for the completion of administrative work in the Intellectual Disabilities program. Work assignments are varied in nature and are performed under the supervision of Operations Director. ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Assembles Yearly Paperwork for all Life Sharing Consumers to be distributed by Program Specialist for signatures and tracks completion by due date
  • Creates new Life Sharing Provider (LSP) Files and files all paperwork
  • Creates and Assembles Site Books, Medical Book, Daily Program Book for new LS Participants
  • Track, collect and file Homeowners/Renters Insurance, Auto Insurance, Driver License Renewal & Physicals, then update Tracking Sheets for LSP and family members
  • Collect, track, scan to T Drive and file Fire Drills and Fire Safety documents such as Annual Fire Safety Training
  • Track, collect, scan to T Drive and file documentation of Furnace Cleaning at LSP Sites
  • Collects and files Daily Documentation Reports from LSPs
  • Scan individual paperwork into the T Drive and file into individual’s books
  • Scan to T Drive and file in LS Consumer Books completed and signed Quarterly Reports and Assessments
  • Scan and upload Medical Appointments into Health Risk Screening Tool (HRST) online portal
  • Gathers and organizes documents needed for yearly Inspection as requested
  • Copy and distribute forms needed by LSPs
  • Creates reports as needed
  • Responsible for meeting minutes of Team Meetings
  • Check and send COH Reports to Rep-payee Technician
