We are currently recruiting LVNs supporting Harris county coverage area including but not limited to; the North West Houston area. We utilize user-friendly Kinnser software and provide our full-time field nurses with state-of-the-art IPADs for their remote work assignments. This allows our nurses the ease of communicating daily electronically. Our nurses have the convenience of commuting daily from their residence to the patients’ locations, based on the schedules our nurses devise. Once a month our field nurses have a case conference at our office in Baytown, Texas.

Essential Job Functions:

Field nurses work under the direction of the RN, assists in identifying the patient's physical, psychosocial, and environmental needs as evidenced by documentation, clinical record, case conference, team report, and evaluations.

Participate in planning and implementing care in conjunction with the RN, in accordance with the Plan of Care.

Assure the continuity of care through delivery of quality patient care.

Provide effective communication to patient/family, team members, and other healthcare professionals as evidenced by clinical notes, case conferences, and communication notes.

Monitor assigned cases to ensure compliance with requirements of third-party payor.

Qualifications:

Education: Graduate of an accredited school of vocational nursing

Current Texas State license as a Licensed Vocational Nurse

Minimum Experience: One year experience as a Licensed Vocational Nurse, clinical care setting; home health is a plus!

Skills: Nursing skills as defined as generally accepted standards of practice. Must have good interpersonal skills.

Transportation: Reliable transportation. Valid Texas driver license and valid auto insurance that means the State of Texas requirements.
