Aventura at Walton Hills is looking for an Admissions Coordinator for our wonderful SNF in Walton Hills, OH. The ideal candidate is one that is looking for a challenge to take on, expect to tackle it, and be rewarded for overcoming it. The ideal candidate will have prior experience as a Nursing Home Admissions Director, and thorough knowledge of all local hospital systems. The role will require coordinating the resident referral and approval process in an efficient manner.

Admissions Coordinator Responsibilities:

• Coordinates tours, admissions, insurance verifications & pre-certs
• Facilities admissions decisions, and ensure positive admission experience for customers and families
• Communicate special needs of new admissions to staff to ensure a smooth transition.
• Understand the admissions process including policy, procedures, assessments & paperwork requirements.
• Maintain knowledge of regulations governing Medicare/Medicaid/Managed Care reimbursement processes.
• Extraordinary customer service to both external and internal customers. External customers would include referral sources, patients, and their families. Internal customers would include supervisors, clinical & non-clinical staff members, pharmacies, and suppliers

Admissions Coordinator Qualifications:

• Prior experience as post-acute and long-term care Admissions Director
• Possess communication, time management, and organizational skills
• Solid computer skills; working knowledge of MS Office
