In-Home Clinician Job Description Summary

The Master’s Level In-Home Clinician provides clinical treatment, support, assistance, and clinical case management to children, youths and families at risk of and/or with a history of psychiatric or substance abuse disorder. Children, youth and families will be self-referred or referred for services by DCF, DMH, other state agency, schools, or other providers.

As an In-Home Clinician you will:

• Be responsible for assessment, diagnostic formulation, treatment and safety planning;
• Provide clinical case management and linkages to community supports, resources, and opportunities;
• Coordinate a multi-modal approach to care including individual, family, and group treatment in the home and other community settings;
• Provide intensive family treatment and support interventions that assist the youth and family to overcome challenges posed by behaviors associated with the youth & family's condition;
• Coordinate care with schools, service providers, and other entities that provide support to assure that the approaches taken are integrated and clinically coherent;

Required education: Master's Degree in Mental Health or Related Human Svcs Field

Required experience: 1 year of Clinical Experience a plus

Required language: Bilingual a Plus!!!
