People, Incorporated is seeking a Maternal Child Health Nurse for the Welcome Family Fall River Program. This position will provide home-based services to participants.
ESSENTIAL FUNCTIONS:
- Work collaboratively as member of small team in providing services to families who have recently given birth.
- Collaborate with referral sources and Charlton Memorial Hospital in maintaining strong partnering relationships.
- Visit families at Charlton Memorial Hospital, presenting this universal program and scheduling visits.
- Conduct up to 90-minute visit at the family’s home or a mutually agreed upon location.
- Assess the family needs and strengths related to overall health and well-being.
- Conduct screenings related to the six core areas: clinical assessment, unmet health needs, maternal and infant nutrition, substance use, emotional health, and domestic violence.
- Engage, assess and present information in an unbiased, culturally humble and appropriate manner, particularly related to sensitive topics.
- Provide screening, brief intervention, and education to all families, regardless of perceived risk or need.
- Address the caregiver’s individual questions or concerns.
- Collect data using the Welcome Family forms, complete documentation and billing with accuracy and according to MA Department of Public Health (DPH) standards in alignment with program policies.
- Offer referrals to services for caregivers with an identified need, including MA MIECHV-funded home visiting programs and other community resources that may best suit the needs of the family.
- Participate in an annual site visit with DPH.
- Complete Agency program and state required training.
- Demonstrate proficiency through experience, education, or training to meet MA Department of Public Health Nurse Competencies.
QUALIFICATION
- Current licensure as an RN by the Massachusetts Board of Registration, Division of Professional Licensure, with either:
- a Bachelor of Science degree in Nursing from an accredited program and at least three (3) years of clinical experience in prenatal, newborn, infancy or maternal services; or
- a Master of Science degree in Nursing with Maternal and Child Health, Family Health, Community Health, or related specialty and two (2) years of clinical experience in prenatal, newborn, infancy or maternal services.
- Proficiency in Spanish required.
- Demonstrate ability to service families with unbiased, non-judgment, cultural awareness, respect and humility.
- Experience with community-based/public health nursing.
- Experience of childbirth, women’s health, newborn care and development, and maternal health.
- Ability to recognize the spectrum of postpartum mood disorders through assessment and observation.
- Demonstrate sensitivity, knowledge and skill for screening and discussion of interpersonal violence, substance use, postpartum depression and anxiety.
- Identify risk factors for child abuse and neglect, and knowledge of mandatory reporting laws.
- Knowledge of community resources to support individuals and families.
- Demonstrate knowledge and skill in family engagement.
- Knowledge of reflective practice, motivational interviewing, and trauma informed care.
- Ability to work and communicate effectively within the Agency and the community.
- A qualifying criminal background check, driving record, and a valid driver’s license required.
