Chisago County Public Health is looking for a Family Home Visiting RN/PHN to join our Home Visiting nursing team. Under general supervision, the selected candidate will be trained to provide family and home visiting services using models designed to serve specific populations, which may include the MECSH Model or traditional home visiting. This position will also Provide in-depth health assessments and education centered on pregnancy, postpartum, breastfeeding and parenting issues and concerns. Using nursing process and assessment skills, they will develop an individualized plan of care to facilitate services related to children"s growth and development while supporting their caregivers. This nurse will also provide parenting education, anticipatory guidance, health and safety education, and assistance to families in accessing needed services and resources. Public Health also works with schools, local hospitals and clinics, county social services and other agencies in coordinating care and providing public health nursing services to communities, families, and individuals. Required/minimum qualifications: Complete High School or equivalent; Bachelor of Science or Arts in Nursing- BSN or BAN; Certification as a public health nurse in Minnesota and current licensure as a registered nurse in Minnesota or will receive within 3 months of application.

Provides in-depth health assessment and education centered on pregnancy, postpartum, breastfeeding and parenting issues and concerns. Using nursing process and assessment develops an individualized plan of care to facilitate services related to children"s growth and development while supporting their caregivers. Provides parenting education, anticipatory guidance, health and safety education, and assistance to families in accessing needed services and resources. Works with schools, local hospitals and clinics, county social services and other agencies in coordinating care and providing public health nursing services to communities, families, and individuals. ESSENTIAL DUTIES AND RESPONSIBILITIES

I. Assess Client"s Physical, Social/Emotional and Mental Health Condition

A. Collect Client Health Data

1. Complete Family admission data base to obtain social/physical history and physical assessment, including immunization status.
2. Complete all MCH forms per program area.
3. Identify situations that are potentially life- threatening.
4. All documentation is in an EHR.

B. Take and record client's general physical measurements such as height, weight, temperature, pulse, respiration, and OFC (head circumference).
C. Perform routine assessments and screenings related to infant/child growth and development, women who are pregnant or postpartum and newborns.
D. Obtain specimens as needed; blood sample via finger stick for lead or hemoglobin testing, and pregnancy testing.

II. Develop Care Plan

A. Utilize Omaha System for care plan development including goals related to knowledge, behavior and status for each identified problem area.
B. Formulate care plan with the client.
C. Modify care plan in response to changes in client"s/families" status.
D. Determine appropriate interventions to address potential and actual problems.
E. Care plans follow the recommendations per model requirements, such as Healthy Families America Standards or agency expectations.

III. Implement and Evaluate Care Plan

A. Coordinate delivery of care.
B. Work collaboratively with client and others, as appropriate, to achieve goals outlined in the care plan using strength-based goals.
C. Make appropriate agency and community referrals.
D. Perform emergency/necessary life-saving measures such as CPR or first aid.
E. Assess and evaluate for changes in client/family and change care plan accordingly.
F. Use evidence-based curriculum, such as Growing Great Kid"s (GGK) as appropriate.
G. Make recommendations to other service providers or professional staff for changes in client"s care plan.
H. Review and maintain safety in the home and of infant and children.

IV. Perform Communication Activities

A. Refer clients to internal/external resources.

1. Determines if client should be referred to an internal/external resource.
2. Recommends to the client an internal/external resource that will provide appropriate services.
3. Provides case management services

B. Documentation

1. Utilize the Omaha system of charting to document data related to client assessment, care plan development, implementation and evaluation, and interventions.
2. Provide written reports of care to health care provider as needed.
3. Write discharge summary based on assessment, visits and documentation.
4. Provide data to complete grant requirements on Chisago County PH grants received by Family Health Program.
5. Provide data to other community partners as needed.
6. Complete minor parent form and have available to Chisago County Human Services
7. Document in the electronic health record as required.

C. Travel (travel time only)

1. Travel to client"s home to provide family health care services.
2. Travel to clinic, inspections, or meeting sites to provide services or attend meetings.
3. Deliver and/or pick up supplies, equipment, etc. to and from home settings, schools, community service organizations, etc.

V. Perform administrative activities.

A. Conduct or attend meetings, seminars and special functions.
B. Staff cases with supervisor and/or MCH Team.
C. Follow policies and procedures of the department.
D. Participate in public health activities such as the county fair, health fairs, education classes at the schools, special projects and other community events.
E. Participates in reflective practice supervision on a weekly schedule in accordance to HFA standards as required.

VI. Conduct or participate in training and/or educational opportunities.

A. Attend classes, courses, seminars, workshops and participate in on-the-job training.
B. Share information with appropriate staff regarding the training received.
C. Read publications and current information regarding practices and the nursing and public health nursing profession.
D. Complete training in Incident Command (ICS) 100,200,700,800.
Desired Minimum Qualifications

a. Complete High School or equivalent.
b. Bachelor of Science or Arts in Nursing- BSN or BAN.
c. Certification as a public health nurse in Minnesota and current licensure as a registered nurse in Minnesota.

h. Employee must have demonstrated ability to work effectively and efficiently in a wide variety of computer programs and systems.
i. Must be able to maintain positive working relationship with County Attorney, County Administration, Department Administrative team, and other county and community committees, State offices of the State of Minnesota for miscellaneous programs.
j. Demonstrates adherence to and knowledge of HIPAA and Data Practices in work assignment.
