CARE MANAGER/MEDICATION AIDE

BASIC FUNCTION
The Care Manager/Medication Aide is responsible for providing residents with supervision and assistance, as needed, with the activities of daily living. This may include help with ambulation, bathing, dressing, eating, grooming and other personal hygiene activities, depending upon the needs for the resident and the services offered by the facility

DUTIES & RESPONSIBILITIES
1. Specific job duties include providing personal supervision or assistance with: a. Ambulation: Helping residents’ maintain balance when getting in and out of bed and providing necessary help with walking or manipulation of a wheelchair. b. Bathing: Assembling towels, soap and other necessary supplies, helping the resident in and out of the shower/bath, turning the water on and off, adjusting water temperatures, washing and drying portions of the body which are difficult for the resident to reach and being available while the resident is bathing. Maintains residents’ shower/bath schedules under the direction of the Resident Services Manager. All residents are scheduled for showers/baths and shampoo on a regular basis.

c. Dressing: Helping the resident to choose and to put on appropriate clean clothing, fastening hooks, buttons, zippers and ties. d. Grooming: Helping the resident shave, brush teeth or clean dentures, wash, comb and curl hair, clean, cut and file fingernails and toenails. e. Beds: Making beds throughout the building when needed. Changing of beds weekly. f. Eating: Escorting residents with walkers and wheelchairs to and from the dining room as needed. Helping with cutting food. Helping with supervision of the residents’ in the dining room as assigned. Feeding residents’ that aren’t able to feed themselves. g. Toileting: Assisting with toileting needs such as getting on and off the toilet and applying personal continent protection such as adult briefs. Assisting with hygiene as needed. h. Supervision: Guiding residents, being aware of the residents’ general whereabouts, monitoring his or her health, safety and well-being, reminding the resident to carry out daily living activities or appointments as appropriate. i. Providing Emotional Security: Promoting feelings of safety and freedom from danger, fear and anxiety. j. Preparing Residents for Bed: Giving evening showers/baths when scheduled. k. Transport: Arranging transportation to medical/dental appointments. Assisting residents with making medical/dental appointments.
2. Observe and assess residents’ conditions and needs.
3. Report any changes in the residents’ conditions and needs
4. Document observations on the flow sheet according to established charting procedures as needed.
5. Maintain a working knowledge and ability to execute the disaster preparedness and safety policies and procedures.
6. Perform the additional responsibilities to assist residents with activities: a. Prepare snacks and drinks. b. Work with volunteers who are present to help entertain residents. c. Help exercise residents through planned activities such as group walks and other outdoor activities.
7. Attend all staff meetings and in-service programs as assigned.
8. Administrative tasks: a. Compliance audits. b. Attend resident care meetings.

9. Medication Aide responsibilities: a. Understands and practices confidentiality of information about the resident. b. Recognizes the resident’s right to refuse medication. c. Follows current standards of hygiene and infection control when handling medications. d. Follows safe handling, storage and disposal of medications. e. Follows safety in application of medication procedures. f. Recognizes and report unsafe conditions for medication provision. g. Accurately documents medication provision (including controlled drugs). h. Provides medications according to the Six Rights.

i. Provides medications according to individual resident needs and abilities. j. Recognizes and reports general reactions to medications. k. Safely provides medications for all ages by the appropriate routes. l. Recognizes the limitations of the Medication Aide role. m. Recognizes and knows how to report suspected abuse and neglect. n. Recognizes the recipient’s property rights and physical boundaries. o. Clarifies any order or treatment regimen believed to be inaccurate, ineffective or contraindicated by consulting with the appropriate licensed nurse (RN/LPN). p. Notifies the appropriate RN/LPN when making the decision not to administer medication or treatment. q. Knows the rationale for the effect of medications and treatments and correctly assists with self-med administration. r. Accurately reports and documents the resident’s symptoms, responses and status. s. Monitors residents who self-administer medications and reports deviation from the plan of care to the licensed or registered nurse. t. Assists with medications within 60 minutes of scheduled time. u. Secures medication cart. v. Records the assist with medication on the Resident’s medication sheet at the time the medication is given. w. Reports a drug assist with error or drug reaction to the appropriate RN/LPN. x. Takes and records vital signs upon assist with medications when required. y. Observes, follows and correctly performs manufacturer’s recommendations including: i. Shake well. ii. Roll or mix. iii. Do not crush. iv. Adequate fluids. v. Medication taken with food or antacids. vi. Instillation of eye medication. vii. Medication via metered dose inhalers. z. Removes discontinued, outdated or deteriorated drugs from inventory.
10. Communicate proficiently via telephone, computer and in person.
11. Practice Universal Precautions.
12. Encourage and monitor safe work practices.
13. Provide additional safety and oversight of residents’ as needed.
14. Facility is in operation 24 hours per day, 365 days per year. At times, this position may have to work outside their normal work hours to meet the needs of the facility.

SKILLS AND REQUIREMENTS
1. Must possess good communication skills.
2. Must possess working knowledge of Universal Precautions.
3. Must be creative, sensitive, responsible, patient and tolerant.
4. Must be able to help maintain a calm environment.

PHYSICAL DEMANDS
1. Regularly stand, walk, use hands and smell.
2. Occasionally reach, climb or balance, stoop, kneel, crouch, and crawl.
3. Must be able to move, turn and lift residents as necessary and push wheelchairs.
4. Must be able to assist residents from floor to feet with the aid of another staff person.

WORK ENVIRONMENT/EXPOSURE TO ENVIRONMENTAL CONDITIONS
1. Work near moving mechanical parts, in high or precarious places.
2. Ability to work around fumes, airborne particles, blood borne pathogens, infectious disease, toxic or caustic chemicals occasionally.

EDUCATION & TRAINING
1. CNA/HHA or equivalent experience preferred.
2. Current Med Tech certification.
3. CPR and First Aid certified

EXPERIENCE
Professional experience in the practice of geriatric Resident Services.
