Memory Care Unit Coordinator/CNA Job at Emerald Nursing & Rehab Cozad

Job Description
- This position entails 3 days in the Memory Care Office and 2 days in a CNA role.
- Coordinates the development of the unit objectives, policies and procedures
- Prepares and provides executive director with records and reports regarding the unit’s budget and progress toward stated goals
- Completes related assessments and tools for admission, and discharge
- Coordinates, with team members, the development of the 24-hour plan for the residents and quarterly updates in the progress notes. Assures delivery of service for each resident according to his or her 24-hour plan. Develops a resource file to assist in meeting the resident’s families’ needs
- Acts as a liaison facilitating communication between the unit, facility, councils, family members and community
- Provides for a therapeutic environment in the residents’ personal and congregate areas, compensating for cognitive, sensory and physical impairments
- Completes all other job duties as assigned by the Direcrtor of Nursing
Requirements
- Must have a CNA license or the ability to attain one. We can help with the CNA requirement.
- Experience with dementia/Alzheimer's Disease
- Experiance in social work, psychology, gerontology, nursing, or therapeutic recreation preferred but not required
- Must be capable of maintaining regular attendance.
