What does an Integrated Care Case Manager do?

Case Managers establish trusting relationships with individuals in order to support efforts to accessing resources and achieving goals. These goals can include wellness and prevention education, coaching, chronic disease management and navigating complex systems of care. Case Managers also assist with coordinating care for individuals, including assisting people with appointments, treatment plans, or referrals for other services. The Integrated Care Case Manager is part of our Vail Care Behavioral Health Home team as well as works alongside staff across Vail Place to meet the needs and goals of individuals served.

Essential Duties and Responsibilities:
The following duties are normal for this position. These duties are not to be construed as exclusive or all-inclusive.

  • Establish trusting relationships with individuals and engage individuals and identified formal and informal supports in shared decision making.
  • Educate & support individuals in accessing community resources and services to manage care gaps and barriers to achieving optimal health.
  • Act as individual advocate and liaison with team members and outside providers, and support adjustments to treatment goals in response to changes in status
  • Coordinate care with individuals to ensure they can successfully navigate their health care needs and goals.
  • Coach individuals and their identified supports towards health literacy through wellness and prevention education, chronic disease management and navigating complex systems of care.
  • Provide crisis assistance and intervention, including use of community crisis services.
  • Assist individuals with appointments to build skills necessary for independence in doing so.
  • Share a caseload (generally between 45-50, flexes based on programmatic needs) with the Behavioral Health Home team members.
  • Meet standards for face-to-face and other contact with individuals and supports. Complete timely and accurate documentation of required paperwork and case notes as required by performance standards.

Minimum Qualifications

  • Must meet the requirement of either A, B, C or D:

A. Holds a certification of a Community Health Worker; or
B. Holds a bachelor’s degree or has completed at least 30 semester credits or 45 quarter credits in behavioral sciences or related fields; AND:

  • Has at least 2,000 hours of supervised experience in the delivery of services to persons with mental illness, substance use disorder, emotional disturbance, traumatic brain injury, or developmental disabilities, OR
  • Has completed a practicum or internship that (1) required direct interaction with adults or children served, and (2) focused on behavioral sciences or related fields.

C. Has at least 4,000 hours of supervised experience in the delivery of services to persons with mental illness, substance use disorder, emotional disturbance, traumatic brain injury, or developmental disabilities.
D. Holds a master’s or other graduate degree in behavioral sciences or related fields.

  • Must have and maintain a valid Minnesota Driver’s License, maintain automobile coverage and have access to an automobile

Work Environment: Keeping us Safe
In response to the COVID‐19 Pandemic, Vail Place has worked diligently to establish strong health and safety protocols for our community. This position is typically a face-to-face, in-person role. Currently many job duties can be fulfilled by phone or video-assisted technology, but some situations will require in-person meetings. This job is primarily based in the Minneapolis/St. Paul area. Any interactions that require in-person contact will involve the use of social distancing and personal protective equipment. Vail Place grounds are also tobacco free to promote health and wellness for the individuals we serve and for our employees.

Salary and Benefits
$44,500-$49,500 Depending on Qualifications. Full Benefits Package including medical, dental, vision, life, AD&D, long term disability, matched 401K. Paid time off in Year 1 includes generous vacation & sick/safe time, plus 3 personal days and 13 paid holidays.
