MSW – Care Resources, full-time, Mon-Fri days!
Care Resources is seeking a full-time Social Worker (MSW) to add to this team as it continues to grow! This position works under the supervision of the Social Work Supervisor and is a critical member of the interdisciplinary team. The MSW provides direct social work case management services to participants in the Care Resources Programs.
Qualified candidates will have a valid MSW license in good standing in the State of Michigan as well as a minimum of 1 year of working with the frail or elderly population. A valid driver's license and reliable transportation are necessary to make the home visits, mostly within Kent County.

ABOUT CARE RESOURCES:
Care Resources operates the PACE program (Program of All-inclusive Care for the Elderly) which is located in our beautifully renovated facility at 4150 Kalamazoo Avenue SE in Grand Rapids. Care Resources provides and coordinates all medical care and social services to eligible participants. We provide all physician and nursing care, prescription drugs, durable medical equipment, dental, eye and foot care, hospital care, medical transportation, home care, emergency, rehab, social and recreational activities at the Day Center. In order to be eligible for the PACE program, seniors must be 55 and older, able to live safely at home and meet the nursing facility level of care.
