The Manager for Case Management Services reports to the Case Management Director and is responsible for promoting patient-centered care by coordinating all aspects of hospital based case management for his/her assigned are of responsibility in alignment with the goals of the Case Management Department. The manager is accountable for the overall day-to-day oversight and management of the case management program including coordination, supervision, and administrative oversight of the case management team. the Manager also functions as an expert clinical practitioner, case management subject-matter expert, resource, advisor and leader for the members of the case management team. Also, the Manager supervises and monitors professional and support staff and ensures that effective care coordination and case management practices are consistent with hospital policies, and applicable regulations and guidelines. Other duties include monitoring quality of the interactions, documentation and assessments of the case management staff, and coordinating activities that promote quality outcomes and patient throughout while supporting a balance of optimal care and appropriate resource utilization.

Job Summary and Qualifications

The Manager Case Management is responsible for supporting the facility by ensuring effective and efficient utilization review and discharge planning services to all patients, as needed. In this position, you will oversee and manage the Case Management Department functions and staff. What qualifications you will need:

• Licensed as a Registered Nurse in the State which you practice or must hold Compact/Multi-State RN License required
• BSN required
• 3+ years of experience in clinical nursing required
• 2+ years of acute care setting case management experience required
• InterQual experience preferred
• Certification in Case Management preferred
• Certification in InterQual or IQCI preferred
