The nurse practitioner will support the North Carolina - Psychiatry Access Line (NC-PAL) and is responsible for providing direct patient care services, collecting and documenting data, providing professional consultations, conducting diagnostic and therapeutic procedures, ordering and scheduling laboratory studies, and supporting the medical and nursing team’s ability to provide services to patients. Specifically, the nurse practitioner will provide behavioral health assessment and consultation services, care plan development and facilitation, and referral to appropriate levels of care. The role functions as an integral part of an interdisciplinary team, ensuring excellence in clinical consultation, referral support, and transitions of care to achieve optimal clinical outcomes. The nurse practitioner will interface directly with clinicians and/or support services staff within Duke Health and in the community, with the goals of supporting patients, families, and clinicians regarding access to behavioral health resources, addressing systemic barriers to mental health, and providing education and consultation to medical and nursing professionals.

Description



Direct clinical assessment and intervention


  • Perform diagnostic and therapeutic procedures to assist the behavioral health team in diagnosing medical and behavioral health problems and prescribing necessary treatment and services for high quality patient care
  • Take patient histories, conduct physical examinations, write progress notes, prepare patient workups and clinical summaries noting pertinent positive and negative findings, obtain psychosocial history, and assume responsibility for developing therapeutic relationships with patients; this may include ordering and scheduling laboratory studies and diagnostic procedures and explaining the necessity, preparation, and anticipated effects of such procedures to the patient and family
  • Perform targeted interventions to assist patients with connection to health care resources and services
  • Monitor quality and effectiveness of interventions by setting long- and short-term specific, measurable goals
  • Participate in case reviews and multidisciplinary health care team assessments
  • Electronically document all activity in the electronic medical record, and in other documentation systems relevant to the program


Clinical consultation and coordination


  • Facilitate interdisciplinary communication to include specialists, PCP, RN, behavioral health, and other key providers
  • Support the behavioral health consultant in responding to calls from the NC-PAL phone line, including through provision of behavioral health consultative services, referral and care management, and urgent/emergent needs
  • Provide education to clinicians (ex. PCPs, NPs, PAs), clinic staff, community agencies, and/or patients and families regarding behavioral health, including through support of consultation program (e.g., completing e-consults) and through participation as a facilitator of staff training/consultation
  • Participate in regular meetings of multidisciplinary health care team


Staff training, supervision, & professional development


  • Participate in the training and supervision of behavioral health consultants (nursing, social work, counseling)
  • Remain current with health care developments and relevant medical/legal issues that impact on patient care
  • Participate in department, hospital, and medical center continuous quality improvement efforts, regular supervisory conferences, and other professional development efforts identified in conjunction with supervisor(s)


Outreach


  • Develop and maintain positive relationships with customers internal and external to Duke Health system
  • Engage and inform clinicians and/or support services staff within Duke Health and in the community about NC-PAL
  • Maintain working knowledge of and liaison with community agencies and resources


Program development


  • Work closely with program administrative core to continually evaluate and improve program implementation activities
  • Attend and participate in staff, committee, department, and other administrative meetings.

Education/Training

  • Work requires completion of accredited Nurse Practitioner program. State of North Carolina licensure is also required.
