Ready to join? Weare looking for a full-time Occupational Therapist to join our rehabilitation team. The position will be based at our Naperville, IL facility with frequent travel to our locations in Joliet, IL, Orland Park, IL, Park Ridge, IL, Westchester, IL, and Oak Brook, IL. Our OT’s plan, implement and evaluate patient care programs designed to increase, maintain or restore function, minimize or prevent disability and assist patients in reaching their maximum performance levels.

Responsibilities:

  • Evaluates patient and gathers appropriate information to determine accurate clinical diagnoses and treatment plan.
  • Establishes appropriate, functional and objective goals based upon clinical examination.
  • Devise detailed treatment and care plans for patients.
  • Provides exceptional patient care and service.
  • The ability to maintain friendly, cordial relations with all clients and employees; maintain a positive work atmosphere by acting and communicating in a manner that results in positive and respectful working relationships.
  • Cultivate relationships with patients and the community.
  • Provide continuous communication with referring physicians and other referral sources to ensure a consistent level of care.
  • Maintain patient charts, clinical logs and checklists necessary for compliance.
  • Maintains patient confidentiality with regard to medical records per company policies and procedures and assures compliance with all HIPAA guidelines.
  • Communicates well in a professional setting.
  • Perform additional duties as required.

Education and/or Experience

  • Bachelor’s, Masters or Doctorate Degree from an accredited Occupational Therapy Program
  • Current State of Illinois Occupational Therapy license
  • CPR Certified
  • Minimum 5-7 years experience (preferred)
  • CHT (preferred)
