We are looking for licensed OT/OTA/PT/PTA professionals to care for our patients. As an OT and PT you will plan, implement and evaluate patient care to restore or maintain patient well being. As an OTA and PTA you will work with the OT and/or PT. The ideal candidates are responsible and well-trained professionals able to give the best nursing care with little supervision. You will be compassionate and cool-tempered. You will be able to follow health and safety guidelines faithfully and consistently. The goal is to promote the patient’s well-being by providing high quality nursing care. This is a contract PRN position with competitive rates and a flexible schedule.

  • Northwest (Includes but is not limited to: Tomball, Magnolia, Montgomery, The Woodlands, Lake Conroe, Cypress and Waller)
  • Northeast (Includes but is not limited to: Channelview, Porter, Spring, Cleveland, Dayton, Crosby, New Caney, Baytown, Humble, and Conroe)

Qualifications

Education:

  • OT - Bachelor of Science in Occupational Therapy
  • OTA - Graduate of an accredited school of occupational therapy assistant with an Associate Degree in Occupational Therapy
  • PT - Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program
  • PTA - Graduate of an accredited school of physical therapy assistant with an Associate Degree in Physical Therapy

Licensure/Certifications/Insurances/Requirements:

  • Current State license as an OT / OTA / PT / PTA
  • Current valid Driver's License
  • BLS Certification
  • TB results within last 2 years
  • Professional Liability Insurance required

Experience/Skills:

  • OT / OTA / PT / PTA in a clinical care setting, hospice or home healthcare of two years is preferred; one year is required
  • Therapy skills as set forth in the Therapy Practice Act and other regulatory agencies
  • Knowledge of durable medical equipment

Transportation:

  • Travel required, must have reliable transportation and valid auto liability insurance

Environmental and Working Conditions:

  • Works in patients home in various conditions
  • Possible exposure to blood and bodily fluids and infections diseases
  • Ability to work flexible schedule including PRN emergency call

Physical and Mental Effort:

  • Prolonged standing and walking required, with ability to lift up to 50 lbs and move patients.
  • Requires working under some stressful conditions to meet deadlines and patient needs
  • Make quick decisions and resource acquisition
  • Meet patient/family individualized psyco social needs

Essential Functions:

  • OT / PT - Perform clinical assessment and identify patient need and oversees the OTA/PTA, when applicable, in the performance of patient care that is consistent with patients needs and desires
  • PT - Plan and provide appropriate treatment for neuro-muscular, neurological and orthopedic disorders based on medical referral and therapy assessment
  • OTA - Administers the occupational therapy treatment plan as developed by the OT
  • PTA - Performs services planned, delegated and supervised by the PT
  • OT / PT - Document, prepare, revise, and complete a Plan of Care to include progress notes, phone orders and any other clinical record documentation on an ongoing basis
  • OT / OTA / PT / PTA - Accept clinical assignments that are consistent with education and competence to care for patients.
  • OT / PT - Apply concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease
  • OT / OTA - Assess and facilitate through direct therapeutic intervention patients ROM, strength, coordination, need for and use of adaptive equipment and environmental modifications, functional mobility and safety issues as related to patients ADL performance needs. Fabricate and maintain adaptive equipment and hand splints.
  • OT / OTA / PT / PTA - Uses effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
  • OT - Evaluate patient occupational performance in the areas of self-care, work, leisure and rest to include sensory-motor neuro-muscular, psycho-social and cognitive-perceptual components of occupational behavior and physical, social and cultural components of the patients occupational environment
  • PT - Assist/instruct patient/ family in transfer techniques appropriate to patient abilities. Instruct on use of equipment such as walkers, wheelchair, canes, etc., in a safe manner. Utilize appropriate therapy equipment specific to patient diagnosis and needs
  • OTA - Promotes compliance with all fiscal intermediary and/or other third party payors, through education, coaching, and other assistance as necessary
  • OT / OTA / PT / PTA - Meet mandatory continuing education requirements of the agency/licensing board.
  • OT / OTA / PT / PTA - Demonstrate commitment, professional growth and competency.
  • OT / OTA / PT / PTA - Promote Agency philosophy and administrative policies to ensure the highest quality of care
