Primary Job Responsibilities:

  • Perform palliative care utilizing strong assessment and evaluation of the patient and their families
  • Collaborate with the patient's physician, other health care team members, the patient and their family in developing realistic care expectations based on goals of care
  • Facilitate patient and family decision making regarding treatment decisions
  • Recommend pharmacological and non-pharmacological interventions
  • Monitor patient's response to treatment modalities and revise plan as needed
  • Assist in all facets of care coordination for palliative care referrals
  • Communicate effectively with patients and families for understanding of diagnosis, prognosis and treatment plans
  • Serve as a resource to healthcare team members regarding palliative care and end-of-life care management
  • Develop and maintain effective working relationship with providers, staff, leaders and community organizations to promote positive image; heighten awareness and promote collaboration
  • Conduct encounters and complete documentation as required in a timely manner
  • Community based position: providing palliative care to patients wherever they call home
  • Graduate of an accredited school of nursing and nurse practitioner program
  • Current license in good standing to practice in the Commonwealth of Pennsylvania
  • Minimum of two years RN or NP experience
  • ACHPN certification required or be able to obtain certification within 6 months of hire
  • Maintains clinical and professional competence
  • Possess a controlled substance license in the Commonwealth of Pennsylvania
  • Possesses the ability to communicate effectively with all members of the care team
  • Possesses good organizational skills
  • Current Pennsylvania driver's license
  • Reliable transportation and automobile insurance required

The Palliative Care Nurse Practitioner provides consultation in palliative care, symptom management and supportive care to meet the needs of the patient as requested by the referring physician. The nurse is a specialist in assessing the patient's condition and developing a plan of care to improve, monitor, and document the course of treatment prescribed by the physician for the patient's diagnosis/prognosis. The plan of care is developed to reduce/manage symptoms, educate the patient and caretaker, facilitate clarification of patient and family goals of care, and assist the patient/caretaker in deciding next steps of care depending on the individual needs of the patient. Additionally, the nurse communicates with all members of the care team to insure proper coordination of care.
