Job Title: Part-Time Shelter Case Manager

Position Summary: The Part-Time Shelter Case Manager is responsible for individual case management services including, but not limited to: assisting in the securing of permanent housing, resource referral, goal setting, advocacy, coordination of services, and networking for single adults and families who are residents of Willis Dady Emergency Shelter. Sound judgment, collaborative style, ethical conduct and motivational interviewing or similar techniques are essential to success in this position. The position is grant funded through September 2023 and further contingent upon grant renewals.

Principle Responsibilities/Duties

  • Assist Shelter Services team to meet shelter goals so at least 70% of clients exit shelter to permanent housing within 30 days.
  • Work with each client to establish and execute a housing stability plan within 48 hours of shelter entry.
  • Meet with each client a minimum of once per week to facilitate connections to resources such as employment, financial assistance, counseling, and education in order to support clients in obtaining and maintaining long-term housing.
  • Collaborate with other case managers to explore housing options and coordinate resources and referrals.
  • Discuss employment and ongoing income with each client, to formulate action steps that can help clients obtain stable income and housing.
  • Advocate for clients as needed to help establish housing and ongoing income.
  • Incorporate a housing first and “rapid re-employment” approach to working with shelter clients.
  • Coordinate and openly communicate with other agency case managers and staff.
  • Assist Shelter Services team to research and implement best practices in a low barrier shelter model to best meet the needs of our clients.
  • Provide case management and links to community resources to ensure over 50% of clients maintain long-term housing stability after exit
  • Utilize a trauma-informed care and housing first approach when working with all households in shelter.
  • Maintain ongoing positive relationships with housing providers, including private landlords, property management companies, and income-based housing providers. Initiate new relationships with a minimum of six new landlords each year.
  • Provide referrals and warm hand-offs to clients for any needed assistance with basic needs, such as mental health, substance use, and/or physical health.
  • Counsel clients through individual, family, and group interaction; assist in educating clients on financial literacy, landlord/tenant rights, and any other educational material to promote long-term housing.
  • Maintain ongoing, positive relationships with community partners to include homeless service providers, and mental health and substance use providers.

Administrative

  • Maintain shelter record keeping and all communications in a confidential manner, as per agency policy
  • Maintain and update case notes in an efficient and confidential manner, including a weekly update on the housing and employment status of each client.
  • Provide data and reports periodically or as requested by Executive Director, Shelter Services Director and/or Grant Writer.
  • Obtain ServicePoint training, pass proficiency test and maintain status.

Operational

  • Admit, orient and exit clients at shelter as needed.
  • Ensure safety and security of clients, staff and volunteers according to agency policies.
  • Consistently and diplomatically enforce shelter rules and policies.
  • Maintain work space in a professional and welcoming manner.
  • Provide information via telephone, make referrals, screen clients for eligibility, collaborate with outside agencies, and take messages for other staff and clients.

Other

  • Attend staff meetings and work assignment reviews regularly as scheduled.
  • Communicate daily interactions, case developments, issues, violations, concerns as needed to Shelter Services Director and Shelter Manager
  • Apprise staff of any client issues that potentially present a danger to client, other clients, staff or others.
  • Maintain scheduled work hours and notify supervisor of absences.
  • Become certified in Mental Health First Aid, De-Escalation, Trauma Informed Care, and SOAR Certification within 90-days of employment. Attend additional trainings as required.
  • Represent the agency at community and fundraising events as assigned.
  • Demonstrate a positive attitude and professional manner, both in appearance and actions in carrying out job responsibilities.
  • Adhere to all shelter guidelines, policies, and procedures.

Requirements

The Part-Time Shelter Case Manager must be self-driven and motivated to work with people experiencing homelessness. The case manager will work to keep people safe in the shelter and help them move towards long-term housing. The Part-Time Shelter Case Manager must have the following skills and experience:

  • Highly skilled in crisis management
  • Dependability –the case manager must be able to cover all scheduled shifts as set by Shelter Services Director
  • The ability to work with a wide range of people including clients and other service providers.
  • Demonstrated openness and respect for all clients regardless of personal differences
  • Clear verbal and written communication
  • Ability to properly complete all required paperwork
  • Previous experience working with vulnerable populations is highly preferred
  • Ability to handle a high level of stress and successfully de-escalate conflict

Other: must have held a valid driver’s license and personal vehicle.

All Shelter staff and clients are encouraged to wear masks but not required. masks are readily available for staff and clients to use. All offices, surfaces, and common areas are disinfected throughout the day.
