MSW/MFT/Master Level in Social Service Field
East Valley Community Health Center is currently seeking a compassionate and team oriented, full-time MSW/MFT or Master Level Social Service candidate to work as a Patient Care Manager within a team setting to do intakes, assessments, care planning, interventions, education, linkage and referrals to HIV positive clients. Master Degree required. Bilingual in English and Spanish is required Bilingual in English and Spanish is required.
Major Position Responsibilities and Functions:

Work in coordination with the Medical Care Manager for active clients with focus on non-medical case management activities.

  • Re-engage clients fallen out of care to promote the availability, access and support of mental health and risk reduction services to facilitate better HIV medical care and compliance.
  • Intake, screen and track all assigned and eligible active clients.
  • Conduct client assessments and reassessments with focus on non-medical issues, which include risk behaviors, substance abuse, and mental health issues.
  • Develop, monitor and modify client care plans along with the Medical Care Manager and Case Worker based on information obtained during the client assessment. Assist clients with needed referral linkage and education to achieve care plan goals.

REQUIREMENTS:

Masters Degree in one of the following disciplines: social work, counseling, psychology and/or marriage and family
counseling and registration with the appropriate state board, if applicable.

  • Bilingual in English and Spanish preferred.
  • At least one year of professional service related to a specialized program area preferred.
  • Must relate well to all cultural and ethnic groups in the community.
