Currently we are seeking a Patient Navigator for our Wound Care Team at ECMC! This is a full time position scheduled M-F 8-4pm
The purpose of this position is to assist patient and their family members in accessing coordinating healthcare needs related to chronic disease management, cancer screenings and population health.
Essential Functions:
- Works with the population health team and patients to schedule appointments.
- Assists in locating healthcare facilities for patients’ specific needs.
- Ensures all necessary components of the healthcare visit are arranged including prior authorizations, transportation and necessary clinical documentation.
- Meets with patients and families to coordinate care.
- Works as part of an interdisciplinary team geared towards providing complex care management.
- Performs required functions related to the Electronic Medical Record, including but not limited to proper documentation of patient and family interactions.
- Deliver exemplary customer service in order to provide a positive patient experience across the organization.
Qualifications:
- Education: Bachelor’s degree preferred or related experience
- Experience (Minimum Needed): Experience and knowledge of principles, procedures and techniques used in providing care to patients in an outpatient setting.
- Good Knowledge of administrative practices and procedures.
- Good verbal and written communication skills.
- Ability to manage multiple priorities and projects.
- Ability to communicate and work in a team.
