Compensation:
$43,950.40 to $49,649.60/Annual ($21.13 to $23.87/Hr.)*

  • Salary can vary depending upon education, experience, or qualifications. Though this position is unclassified, pay is equivalent to classified Pay Grade 24, Steps 12-17.

Position Summary & Responsibilities

Position Summary: This position will serve as the Care Coordinator for participants involved in the Wichita Kansas Supportive Housing Program (K-SHOP). The K-SHOP program focuses on residents and parolees with severe and persistent mental illness who also experience co-occurring substance abuse as well as a history of homelessness.

The Care Coordinator will identify and screen participants for the program from correctional facilities throughout the State. The Care Coordinator will meet with identified participants within the facilities prior to release to develop a rapport, advocate for facility based programming prior to the residnet's release, and connect the participant with community based resources such as housing, mental health and substance abuse treatment prior to and upon release. This will be accomplished by establishing partnerships with property managers/landlords, Comcare, community based substance abuse providers and other community based agencies/providers, and developing and connecting K-SHOP participants with these agencies as a way to reduce their risk of returning to prison. This position will work with providers, community groups, and KDOC staff to identify gaps and develop strategies for filling those gaps. In emergency situations only, this position may have to be available to problem solve with Comcare Staff, Parole and/or Landlords outside the traditional business hours for K-SHOP participants within the community.


Job Responsibilities may include but are not limited to the following:

  • Serve as the liaison between KDOC and Comcare for the K-SHOP program – Identify residents with severe mental illness, drug and alcohol issues, and periods of homelessness who will be returning to Sedgwick County and meet the K-SHOP criteria.

    • Meet with residents prior to release to assess service needs either in person or via telecommunication options.

    • Work with facility staff to ensure SOAR/substnace abuse screenings (and any other screenings necessary) are completed prior to release.

    • Establish a method for accessing Comcare case information, as needed, for eligibility determination and case management while maintaining proper confidentiality.

  • Assess and identify housing needs for K-SHOP participants.

    • Coordinate with KDOC and Comcare housing specialists for them to assist in locating safe and permanent housing for participants.

    • Be available to Housing Specialists and landlords if housing placement is not successful or if issues arise that require intervention by this person with the participant or landlord.

    • Ensure the apartment is furnished and ready for habitation prior to participant moving in, by working with the Housing Specialists to address needs and locate resources to fill those needs.

  • Immediately upon release, connect K-SHOP participants to community based resources, based on their identified needs and/or goals.

    • This may include but not be limited to mental health, substance abuse treatment, etc. Remove barriers such as transportation that may limit service provision.

    • Provide on-going one-on-one case management for K-SHOP participants who are not receiving case management services through Comcare either because the service has not yet been started or because they do not qualify for Medicaid.

    • Responsibilities may include securing permanent housing, connecting to community-based services, conducting house visits, etc. Record interactions with offenders and collateral contacts in the Athena database.

  • Facilitate multi-disciplinary meetings with community partners.

    • This will include staffing program referrals with Comcare to ensure the participants meets the necessary criteria, staffing cases as needed when challenges or concerns arise, and providing programmatic updates to agency administrators on a quarterly basis.

  • Monitor and track identified quarterly data information with the participant and complete entry of said data in collection tool.

  • Other duties as assigned.

Qualifications

Minimum Qualifications: Bachelor’s degree (B.A.) from four-year college or university in a human services or social science area

  • One year of experience in planning, implementing and monitoring acitivies relevant to the agency's programs
  • One year experience working with correctional residents and/or special needs populations

Preferred Qualifications:

  • Licensed Masters Social Worker.

  • Experience in providing case management to persons with severe mental illness.

  • Training and/ or experience in cognitive programming.

  • Demonstrated skill of problem-solving, multi-tasking and handling a substantial work load.

  • Demonstrated ability to network and connect system pieces/ players on challenging access issues and sustaining collaborative partnerships.

  • Strong organizational, interpersonal and engaging skills.

  • Strong oral and written communication skills.

  • Working knowledge of MS Office Suite.

  • Strong willingness to learn new skills and participate in ongoing learning.

  • Ability to read, analyze and interpret governmental regulations.

  • Ability to effectively present information, respond questions and complaints from widely varied constituencies.

Post-Offer, Pre-employment Requirements: Must possess and maintain a valid Driver's License. Must pass a criminal history records check.
