SUMMARY: Exercises courtesy, compassion and confidentiality, treating all residents with dignity and respect in striving to meet their physical, spiritual and emotional needs.
Performs duties to provide personal care, assisting the residents in meeting their activities of daily living and their instrumental activities of daily living. Provides administration of medications, according to relevant standards. Adheres to medical directives, organizational policies and procedures as well as governmental regulations.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.*

  • Exhibits compassion, interest, and respect for all residents and strives to meet residents’ needs in a courteous, professional manner.
  • Obtains report from the night nurse aide, and communicates with day Team Leader regarding specific daily instructions.
  • Assists residents with personal care, grooming, dressing and hygiene as required by resident’s service level and condition. Oversees residents needing help getting up and dressed, encouraging residents to achieve maximum independence. Assists in transferring and transporting residents according to “no-lift” policy.
  • Provides assistance in the dining room, while overseeing resident safety and well-being. Records percentages for skilled nursing residents and provides transport assistance as needed for skilled as well as PC residents.
  • Serves meals, beverages and other nourishments to residents who are temporarily unable to go to the dining room.
  • Assists with continence management for the residents as needed. Disposes of all incontinence products properly to insure sanitation of residents’ suites and community restrooms using standard care procedures.
  • Provides care and treatments as assigned by the Team Leader, documenting all services rendered; provides restorative techniques on assigned residents, as prescribed by consulting therapists.
  • Observes, reports and documents the appearance of symptoms and/or any physical or mental changes in residents’ conditions in such areas as: skin, weight, mobility, eating and sleeping patterns, behavior, alertness, participation in activities, etc.
  • Responds to security system and resident call bells promptly and immediately. Takes appropriate action including resetting call bells.
  • Maintains orderliness of residents’ kitchen, laundry, tub room, and common areas; maintains resident rooms and residents’ personal items, keeping them neat.
  • Prepares, administers, and records medications according to procedures and protocols and under the direction of the nurse Team Leader.
  • If he/ she is the person who discovers an incident, thoroughly investigates and documents the incident, completes Incident Report on line, notifying all required parties. Notifies the State (DHS) by fax if required. See 2600.16b for the list of reportable incidents. Notifies DHS by fax and by phone call in case which require it. See 2600.16 c.
  • Maintains CPR certification, first aid training and attends required in-services and staff meetings. Maintains Nurse Aide Certification as well as the Med Tech Certification. Attends all mandatory in-services and completes all required on-line training; participates in monthly staff meetings, with 2 excused absences allowed in a year.
  • Consistently delivers excellent service to all our all customers, which includes, residents, family members, visitors and co- workers while meeting or exceeding their needs and expectations. Maintains a positive attitude and is respectful and courteous to all.
  • Adheres to the Peter Becker Community Code of Conduct and all Corporate Compliance, Privacy and Security and HIPAA Policies and Procedures.
  • Attendance is an essential function for this position.
  • Adheres to facility and department dress code.
  • Performs additional duties as required.

EDUCATION AND/OR EXPERIENCE: Med Tech Certification
LANGUAGE SKILLS: Medium level of interpersonal and communication skills; frequent interaction with residents, family members and all levels of staff.**
