Public Health Nurse I or II – Nurse-Family Partnership

This position offers a unique opportunity to work in the public health setting performing a full range of skilled nursing functions as well as administrative duties in support of special program areas meeting grant deliverables. This individual will work within our team of home visiting nurses and will be required to attend Nurse-Family Partnership training.

Responsibilities include:

  • Provide home visits to individuals and their families in Clallam County, who are eligible for the Nurse-Family Partnership (NFP) program
  • Engage in NFP curriculum with families using a strength-based approach
  • Consult and collaborate with other community partners for referring clients to services
  • Assist in outreach on behalf of programming for families
  • Document home visits, perform assessments, engage in data collection, and maintain confidential patient records
  • Work independently with families who may present with complex health and social histories
  • Actively engage in professional development including conferences, trainings, and a mandatory NFP orientation in Denver, CO upon hire
  • Adhere to NFP Model Elements and NFP Visit-to-Visit Guidelines
  • Participate in creating a positive work environment that promotes productivity, mentoring, teamwork, and cooperation
  • Commit to this work for at least three years due to the trainings required in this role

Education and/or Experience:

PHN I: Public Health Nursing Certification from an NLN accredited program and/or a Bachelor’s degree (B.A.) from four-year college or university.

PHN II: Bachelor’s degree in nursing (BSN) from a college or university accredited by the National League of Nursing; and a minimum of two (2) years Public Health nursing experience in a Public Health agency. An appropriate advanced nursing degree or extensive clinical nursing practice in a variety of settings may be substituted for the two (2) years of Public Health Nursing experience. Valid Washington State Driver’s License.

Hours & Compensation:

PHN I is a Grade 33 UFCW position. Full-time/32 hours a week. Pay range is $29.12 - $36.37/hour in 2023, starting pay typically $29.12, DOE. Full medical benefits, including dental and vision. Retirement provided through Washington State DRS. Plus 12 days of vacation, based on hours worked, 11 paid holidays, sick leave, and bereavement leave. Work Week/Office hours: Monday through Friday 9:00 am – 4:30 pm with a 30-minute lunch.

PHN II is a Grade 35 UFCW position. Full-time/32 hours a week. Pay range is $31.36 - $39.17/hour in 2023, starting pay typically $31.36, DOE. Full medical benefits, including dental and vision. Retirement provided through Washington State DRS. Plus 12 days of vacation, based on hours worked, 11 paid holidays, sick leave, and bereavement leave. Work Week/Office hours: Monday through Friday 9:00 am – 4:30 pm with a 30-minute lunch.
