The Physical Therapist provides services to patients who have impairments, functional limitations, and changes in physical function. The Physical Therapist continually assesses the treatment program and patient's discharge planning needs, recommending changes to the program as appropriate. The Physical Therapist interacts and practices with a variety of professionals. Supervises Home Health Aide, PTA as assigned.

1. Regular, consistent, on-site, and timely attendance.
2. Performs clinical responsibilities in a manner that provides effective age-specific patient care while achieving optimal outcomes
a. Completes evaluations within appropriate time frames per policy.
b. Sets goals with measurable outcomes and appropriate time frames.
c. Recommends discontinuation of treatment when appropriate.
d. Initiates discussions with physician and other medical team members when appropriate to alter therapeutic program.
e. Supervises Physical Therapist Assistants and support personnel in a professional manner.
f. Demonstrates appropriate use of various treatment modalities in patient care.
3. Adheres to ethical and legal practice standards
a. Abides by relevant ethical codes and standards of practice guidelines.
b. Abides by State and Federal laws and regulations, including state licensure laws.
c. Maintains records in accordance with department and system polices that is accurate, concise, timely and legible.
4. Maintain records that are accurate, concise, timely, and legible in accordance with Agency and System policies.
a. Documents therapy care in format and accordance with practice setting including: evaluations, treatment provided, response to treatment, patient progress, education, family conferences, discharge planning and discharge summary as appropriate.
b. Completes reassessments at appropriate intervals and documents accordingly.
c. Communicates post-discharge needs to appropriate resources.
d. Documents patient care that is consistent with guidelines and requirements of agencies and third party payers.
5. Educates all patients and families in a manner consistent with their level of understanding and therapeutic needs
a. Involves patient and family members in establishing a plan of care.
b. Provides patient and family members with educational materials as needed.
c. Documents education and training as well as patient and/or family level of achievement with training provided.
6. Capture maximum revenue for quality services provided while maintaining established standards of care
a. Utilizes appropriate documentation including CPT and CDM codes to match services provided.
b. Maintains adherence to established efficiency standards.
c. Completes daily charge entry accurately
d. Provides relevant information to insurance provider to insure proper authorization is secured prior to initiation of treatment (when indicated).
e. Assures treatment does not extend beyond last covered date.
7. Promotes continual performance improvement both within the Rehabilitation department and throughout Excela Health
a. Demonstrates individual initiative for performance improvement both within Barclay Rehabilitation and throughout Excela Health.
b. Actively identifies needs for improvement in Barclay Rehabilitation and system wide and discusses with manager.
8. Collaborates with patients, families, and other health team members to provide excellence in patient care
a. Communicates regularly with other team members via verbal, written, and/or established electronic communication methods.
b. Attends interdisciplinary team meetings when indicated.
c. Attends all required Barclay staff meetings. If absent or unable to attend, reads minutes and signs appropriate sign-in sheets to indicate review of material.
9. Participates actively in the ongoing development of new therapy programs and in the enhancement of established programs
a. Attends appropriate internal or external education opportunities to promote growth and expansion of specific programs.
b. Researches appropriate innovative ides via journal and internet exploration and discusses these with manager leads, and peers.
10. Implements a self-directed plan for professional development
a. Demonstrates an awareness of strengths and weaknesses and seeks guidance as needed to address limitations.
b. Initiates and participates in continuing education: lectures, video/audio conferences, written and online courses.
c. Develops and conducts in-service as appropriate to share information gained from external resources.
11. Upholds fiscal and operational responsibility
a. Utilizes supplies and equipment in a fiscally responsible manner.
b. Maintains adherence to established efficiency standards.
c. Maximizes utilization of non-direct patient care time.
d. Maintains responsibility for reporting accuracy of hours worked.
e. Travel to other entities or sites within the System as patient census dictates, in order to provide quality necessary care where patient need is greatest.
f. Staff schedules require flexibility as they are dependent on patient care needs, which may include evening and weekend hours, as necessary to meet demand in their primary area or the needs across the System.

1. Works collaboratively with the case manager and other agency staff in the provision of patient care. Ensures communication of patient care needs to the primary car staff in order to facilitate continuity of care.
2. Follows established agency guidelines for notification of on call management staff as needed.

  • Minimum 1 year experience in the past 2 years in a hospital, Skilled Nursing Facility, Home Health Agency or other relevant experience..
