Physical Therapist Assistant Job at Horizon Home Health & Hospice
Provide services in conjunction with, or under the direction of a physician, providing patient assessment, patient care planning, direct patient care, teaching, supervision, in-service training, consultation, and administration of therapy.
Work with the physician and other staff in developing a plan of care for the patient and in updating the plan of care as indicated.
Observe, record, and report the patient’s reaction to treatment and any changes in the patient’s condition.
Provide timely notice of plans to discharge the patient from therapy services.
Communicate frequently regarding the patient’s progress and condition.
Submit clinical notes daily including initial assessment documentation, discharge OASIS forms and discharge summaries.
Consult with the physician before changing treatment as outlined on the plan of care or increasing or decreasing visit frequency, and initiate written supplemental orders.
Supervise services provided by qualified therapy assistants (Physical Therapy Assistants) as dictated by patient need, policy and the Arizona State Practice Act. Document all supervision as directed.
Attend and participate in patient care conferences and coordinate provision of services with other health team personnel, the patient, and the patient’s family as appropriate.
