Per Diem/Relief for Leave and PTO. As needed, Monday - Friday. 8:00AM - 5:00PM.

You’ll plan and administer medically prescribed physical therapy treatment for patients suffering from injuries, or muscle, nerve, joint and bone diseases, to restore function, relieve pain, and prevent disability, by performing the outlined duties in an ethical and honest manner and complying with regulatory, departmental and facility policy and procedures.

DUTIES:

Reviews Physician's referral and medical record, obtains pertinent history, and subjective findings including patient goals of treatment and self-perception of disability and pain.

Collects objective findings, including (but not limited to): tests and measurement of patient's strength, ROM, motor development, sensory perception, functional capacity, gait, respiratory and circulatory efficiency, mobility, skin integrity, and balance. Records findings to develop or revise treatment programs.

Completes a computerized summary of the initial evaluation, including as applicable evaluative findings in the Electronic Health Record, treatment plan, short and long term goals, discharge plan, prognosis, and nursing recommendations, assuring a copy is sent to the referring physician. Plan will be inclusive of type, degree and frequency of modalities or procedures to be provided, or that may be provided in the future, as well as duration of planned treatments.

Informs patient of treatment plan developed based on findings and obtains written consent.

Assures all insurance requirements are met for individual case management, with patient informed of his/her responsibilities and all implications, completion of required third party forms and necessary documentation.

Confers with physician and other practitioners to obtain additional patient information, suggest revisions in treatment program, and integrate physical therapy treatment with other aspects of patient's health care.

Performs standard rehabilitative procedures as ordered by physician and indicated by evaluative findings to obtain desired outcomes, within scope of training and competency.

Maintains timely documentation in the form of progress notes, interdepartmental communications, and progress letters to physicians.

Communicates on an ongoing basis with the referring physician to clarify the diagnosis and referrals and notify of any significant changes in patient's condition, and secures all needed components of referral and insurance authorization.

Submits treatment charges daily for each individual treated, assuring required authorization/re-authorizations is obtained and fulfilling any insurance requirements.

Participates in ongoing plan for patient's discharge to assure the patient safe discharge with anticipation of need for adaptive equipment and/or services.

Assists Rehabilitation Services Manager in structured learning experiences for physical therapy interns and BOCES student Practical Nurses, and observational students.

SUPERVISORY RESPONSIBILITIES: Directly supervises Physical Therapist Assistant and Physical Therapy Volunteers in Rehabilitation Services. Responsibilities may include interviewing, training associates; planning, assigning, and directing work; appraising performance; addressing complaints and resolving problems.

REQUIRED EDUCATION and/or EXPERIENCE: minimum of Bachelor's degree (B.A.) .

CERTIFICATES, LICENSES, REGISTRATIONS: Current or pending NYS licensure as a Physical Therapist is required.
