Job Title/Position: Physical Therapist


Job Description Summary


The physical therapist contracted or employed through the organization is responsible for providing physical therapy services and for adherence to all conditions outlined in the Service Employment Agreement.


Essential Job Functions/Responsibilities

  • Provides physical therapy services to patients according to a written individualized plan of care. This may include, but not be limited to:
    • Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment.
    • Assesses for muscle strength, mobility, gait, ROM and transfer capabilities. Less a goal of rehab, more a focus on maintaining comfort.
    • Directing physical therapy treatment.
    • Provides equipment required to increase patient function and independence.
    • Instructing patients and families/caregivers in the use and care of therapeutic appliances.
    • Determining priority needs for physical therapy
    • Reporting to physician patient's reaction to treatment or changes in condition.
  • Initiates physical therapy program and instructs other personnel and/or family/ caregivers in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.
  • May educate patient in the use of prosthetic device.
  • Prepares and submits clinical and progress summaries based on the attainment of goals.
  • Participates in patient discharge planning. Prepares and submits a clinical progress summary based on the attainment of goals directed by organization policy.
  • Provides physical therapy consultation to families/caregivers, as appropriate.
  • Provides inservice education programs for nursing personnel as needed.
  • Participates in the interdisciplinary group.
  • Actively participates in the quality assessment performance improvement teams and activities.
  • Participates in orientation program for hospice team members.
  • Provides supervision of physical therapy assistants and hospice aides, as appropriate.

Position Qualifications

  • Possesses a degree from a baccalaureate or masters program in physical therapy approved by an accredited organization. Meets personnel qualification stated in 418.114(b)(7).
  • Licensed to practice as physical therapist within the state.
  • At least two (2) years of appropriate experience as a physical therapist. Home health or hospice experience is preferred.
  • Demonstrates good verbal and written communication, and organization skills.
  • Possesses and maintains current CPR certification.
  • Understands hospice philosophy, focus of care is comfort vs. rehab and to maintain care in their residence as long as possible.
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Job Responsibilities Include

  • Responsible for the oversight and delivery of quality, patient-centered care rendered in the patient’s residence.
  • Initiate, develop, implement and revise the patient Plan of Care (POC) as needed.
  • Performs a comprehensive physical assessment and history of current and previous illnesses each visit as appropriate including a complete medication review with an appropriate education.
  • Develop, initiate and revise the plan of care, in consultation with the patient, physician/practitioner and other team members as appropriate, which establishes patient-specific goals and incorporates therapeutic, preventative and rehabilitative nursing services as appropriate.
  • Communicates necessary revisions to patient status and/or needs (this includes updating physician facilitating the coordination of services and referrals to other services as appropriate).
  • Regularly consults, advises and reevaluates patient status with patient, family, patient representative and other team members as appropriate regarding safety, plan and outcome of care and discharge planning.
  • Provides complete, accurate and timely documentation consistent with the plan of care

Requirements:

  • Minimum of 1 year of experience as a Physical Therapist
  • Comfortable driving to patient homes for field visits
  • Prior experience in Home Health, Hospice, Rehab, or Home Care or any other role working in patient's homes preferred
