We are seeking a Physical Therapist with strong clinical skills, documentation skills, and a passion for patient care to work on a full-time basis! Home Health experience is preferred, but not required. All applicants must be licensed as a Physical Therapist in the state of Virginia and CPR certified. This position is a Full-Time Position within our service areas of Charlotte County, Lunenburg County, Halifax and other surrounding areas.

Job Requirements Include:

• Licensed as a Physical Therapist
• Graduated from a Physical Therapy program/curriculum approved by the American Physical Therapy Association or other affiliated Associations
• Home Health experience preferred, but not required

Job Duties/Responsibilities Include:

• Evaluates patient's status to determine the plan of care in the patient's home. Considers diagnosis, prognosis, and medical history in determining rehabilitative potential
• Participates in educating the patient and family activities of daily living, safety precautions, and necessary modifications in activity, home, or equipment
• Providing Home Health Physical Therapy services to patients, which includes transfer training, gait training, therapeutic exercise techniques, etc.
