This position is for Wayne County!

This position is contingent and paid on a per visit basis; orientation and additional training will be paid at a separate hourly rate. We offer competitive wages, bi-weekly pay and direct deposit.

Responsibilities:

  • Provide initial and ongoing comprehensive assessments of the patient’s needs, including Outcome and Assessment Information Set (OASIS) assessments
  • Perform assessments within 24-48 hours of receiving the initial referral
  • Establish a treatment plan in consultation with the physician and revises it with the approval of the physician as necessary
  • Assist in development and implementation of the interdisciplinary patient care plan as it pertains to Physical Therapy
  • Educate patients and caregivers
  • Record on a timely basis all evaluation data, treatments, and patient's response to therapy intervention Also records quality assurance data
  • Prepare an assessment of the patient's condition and periodic evaluations and summaries
  • Supervise Physical Therapy Assistants and paraprofessionals as per agency policy
  • Participate with staff, patients, and physicians in discharge planning activities and completes a Physical Therapy Discharge Summary
  • Adhere to Federal, State and Accreditation requirements including Medicare and Medicaid regulations

Minimum Qualifications:

  • Physical Therapy License in good standing within the State of Michigan
  • Preferred Two (2) or more years of PT experience, specifically in a community/home health care setting
  • Must have knowledge of Medicare/CMS guidelines
  • Must have experience with Outcome and Assessment Information Set (OASIS) assessments
  • Experience using an EMR (Kinnser prefer)
  • Must have your own reliable transportation, valid MI drivers license, with proof of auto insurance and vehicle registration
  • COVID-19 Vaccination Required

Pay: $70.00 - $110.00 per hour
