POPULATION HEALTH GUIDE II


SUMMARY OF JOB:

The Population Health Guide (PHG) is responsible for successfully assisting the IWA participating primary care practices in achieving their goals in value based programs. The PHG II will be a practice champion effectuating change in everyday practice procedures, an education resource for specialist and facility utilization as well as assisting the IWA practices in meeting quality and efficiency goals. In working with IWA practices, this role will use data from payor sources, including data in the population health tool as well as practice EMRs to identify individual patient needs as they relate to preventative screens, laboratory testing and general maintenance of wellness. The PHG is adept at a variety of office based EMRs in order to maximize accuracy and efficiency around documenting care, entering value based care metrics and closing of care gaps. The PHG works closely with the Population Health Supervisor, as well as proactively maintains regular contact with assigned IWA practices’ providers and key personnel through office visits, meetings, email and telephonic communication. Further, the PHG is knowledgeable about existing and new Redeemer Health services and offerings and promotes them appropriately to facilitate in system/Tier 1 cost effective utilization. It is essential that this role understand the importance of value based health care and be able to engage patients, to achieve health improvement goals, in defined populations served by IWA. This role will assist in guiding technology utilization (i.e., Office EMR and Population Health Tool) for the advancement and optimization of value based care performance in IWA practices.

RECRUITMENT REQUIREMENTS:

Must have a high school diploma. Advanced degree or education preferred or two years of work experience in a health care setting. All candidates must excel in delivering a positive patient experience, knowledge of health care related terminology, critical thinking skills, proficient computer skills and have excellent communication (both written and oral) skills. This role requires impeccable organizational skills and the ability to work and adjust to different environments. Must be willing to travel to each IWA practice, and be flexible in scheduling visits to physician offices. Must be comfortable with telephonic outreach to physicians, office personnel, and, patients, as well as scheduling, facilitating, and coordinating physician and patient testing appointments. Must be able to use Word and Excel proficiently.
