Job Details:

• PRN Physical Therapist for Hospice Patients
• Territory-Central Indiana and surrounding communities

• PRN as needed

Job Responsibilities:

• Performs accurate and comprehensive assessments within a 5 daywindow from the start of care date.
• Obtains physician orders for certification, re-certification, evaluation and additions to plan of care of home physical therapy services. Orders are entered on the date they are received.
• Ensures that changes in patients’ clinical status are communicated to hospice team members verbally and in writing in a timely manner.
• Coordinates care planning and delivery with the physician, patient and family/caregivers, other hospice team members to facilitate optimal patient outcomes.

Credentials:

• Current driver's license
• Must have reliable transportation and insurance
• Current valid State Physical Therapy license.
• Graduate of an accredited physical therapy curriculum/school.
